This paper reviews the scientific literature in relation to the testosterone. Additionally, it argues about the philosophical and social aspects of this hormone.

Testosterone, gradually, from the embryonic age to the death, shapes the body and mind of a man, forms the individual and then the personality. Testosterone is secreted during the 8th week of pregnancy. Since the 7th month of the perinatal period, the central nervous system starts to differentiate with the male features.

Each organ and system of the in the human body is affected by the testosterone with a certain degree.

Testosterone promotes our creative talent, intelligence, thinking manner, energy improvement.

ASSOCIATION BETWEEN SALT-SENSITIVE HYPERTENSION AND METABOLIC SYNDROME IN THE ELDERLY

It has been known that elderly are a more salt-sensitive subset of the population. Recent clinical studies have demonstrated that MS is an important non-genetic factor that increases the salt sensitivity of BP. Our study was aimed to assess an association between salt-sensitive hypertension and metabolic syndrome in the elderly.

Results of our study showed that in Georgia hypertensive subjects consume high amount of sodium. It appears that in Georgians prevalence of salt-sensitivity exceeds average values with peak levels in the elderly. So, our findings confirm results of numerous studies that demonstrate age dependence of salt sensitivity. Based on our results high sodium consumption in salt-sensitive hypertensive patients of Georgian nationality is closely linked with higher incidence of MS. There is a high prevalence of metabolic syndrome in the elderly, especially in women.

Early detection of MS and salt-sensitivity offers the possibility of change in lifestyle, prevention and treatment of disease.

Rheumatoid arthritis is chronic inflammatory disease, which causes permanent morbidity and mortality of 1% of population.  Polyarthritis is main feature of this disease, but many patients are at a high risk of the development of lymphomas and cardiovascular diseases.

Disregulation of immune system has key role in the pathogenesis of rheumatoid arthritis. Proinflammatory cytokines TNF-α, IL-6 and IL-1 derived by monocytes, macrophages and dendritic cells, recruit and activate  autoreactive Th17 cells in rheumatic joint. These mediators induce secretion of  cytokines IL-17A, IL-22, IL-23, IL-27 from Th17 cells. Their proinflammatory activity has detrimental effect as autoreactive immune response is not regulated by T regulatory (Tregs) cells. Despite increased frequency of Tregs, their functional activity is markedly inhibited.

Thus, impaired balance between pro- and anti-inflammatory cytokines sustains chronic inflammation.  Identification of therapeutic targets among cytokines is of  importance of immune approach in treatment.

CURCUMIN IN COMBINATION WITH DNMT INHIBITOR MODULATES EXPRESSION OF IL-10 GENE IN CYSTIC FIBROSIS

Chronic lung infection with P.aeruginosa and excessive inflammation is the major cause of morbidity and mortality in Cystic Fibrosis. It has been proposed that decreased level of IL-10 may contribute to the excessive inflammatory response in CF lungs. Variable expression of Cystic Fibrosis disease even among individuals with same genotypes suggests that on the clinical manifestation of CF influence another genetic or epigenetic modifiers. The aim of the present study was to investigate potential of natural epigenetic agent, such as curcumin in treatment of CF. 

The study subjects included seven healthy and seven patients with CF (age 5-13 years). All patients had CF diagnosis confirmed by the pilocarpine iontophoresis sweat test as well as by genotyping. The study was approved by the Biomedical Research Ethic Committee of the Tbilisi State Medical University and informed consent was obtained from all participants. 

We showed that simultaneous treatment of CD4+ T-cells with curcumin+5AzaC was more effective than corresponding sequential treatment with curcumin or 5AzaC  in vitro.  Significant effect was achieved with the lowest concentrations of 5AzaC (2.5µM) (p<0.05). In conclusion, this study suggests that anti-inflammatory effect of curcumin+5AzaC  might be at least in part through epigenetic modulation of DNA methylation of IL-10 gene promoter in CD4+ T cells. Further studies are needed to confirm the therapeutic efficacy of epigenetic agents in treatment of Cystic Fibrosis.

PECULIARITIES OF POTENTIAL BIOMARKERS (PROLIPHERATION-APOPTOSIS-STEM CELL) OF CERVICAL NEO

About 84% of cervical cancers occur in relatively less developed countries. In Georgia, cervical cancer is the second most common malignant disease amongst females, after breast cancer, according to data of  2014. Only 10% of HPV infected women show persistent infection, from which 50% develop H-SIL or carcinoma. Therefore, HPV testing is recomended only after the age of 30. Despite the high sensitivity of combined scrining tools, neither cytology and/or histology, nor HPV DNA test can reliably detect the progressive disease. Potential markers of cervical intraepithelial neoplasia progression are proliferation markers Ki67, p16; apoptotic markers CD95, caspase 3, 9, 6; and stem cell markers ABCG2, ALDH1, CD133, CD49f, OCT4, osteopontin, SOX2, CD44.

Even though extensive research efforts are directed to the understanding of cervical precancerous and cancerous lesions, this subject still requires further discussion and more presice investigation, especially understanding the mechanisms of CIN1-CIN3 progression into carcinoma is of high interest, as it has a crucial importance in clinical management of these lesions.

The facts given in this work represent the results of research on subtropical cultures (raw and ready materials). The ways are found to regulate bioactive mixtures consistence of researching plants. The plant is introduced as one of the strongest guaranty of preventing a lot of diseases and successful treatment as it’s less characterized for medicines produced by raw materials to have side affect.

It’s natural that during methodological selection it’s possible to achieve results with the knowledge of classical methods and producing level. To achieve the desirable result is depended on selectionist’s qualification level.

THE USE OF TONSILGON IN THE TREATMENT OF PATIENTS WITH CHRONIC TONSILLITIS AND AFTER TONSILLOECTOMY

Research was made on 380 patients, surgical operation was made because of chronic inflammation and hypertrophy of adenoids. 217 patients had complains referred to general and local tonsillectomy. 163 patients diagnoses were chronic tonsillitis and adenoids, they made adenotonsilectomy with general anesthesia. After two days from operation patients used only solution of tonsilgon, the appropriate dose of age. They have not used in this period any other drugs. Besides in this research attempted controlled 30 patients, which had chronic tonsilitis. They had made bilateral tonsilectomy with local anesthesia, but after operation they were not treated by tonsilgon drops. After our research we recommendate use of tonsilgon drops (10-15days), in an patients which had made operation of adenotonsilectomy and tonsilectomy, which will reduce post operational complications and will reduce this period.

STUDY OF NEUROPATHIC REFRACTORY PAIN

Adequate management of refractory pain developed in neuropathic chronic pain is one of the most difficult and debatable problems in the modern palliative care. An experimental study on refractory pain was conducted in a number of palliative clinics during 1/01/2017-1/06/2018 on 81 randomly selected patients with diabetic and post-herpetic neuralgia, 59% of which participated in the previous double-blind randomized controlled trial in which pregabalin was used in different dosage regimen. Short-Form McGill Pain Ques–tionnaire (SF-MPQ) along with VAS were employed.

The patients were enrolled based on the screening criteria – chronic pain intensity minimum 40 mm by VAS. 90% of the study subjects were classified as refractory to gabapentin and tricyclic  antidepressants. 63% suffered with severe pain. Pregabalin was used in a fixed dose of 150-600 mg/day, that was changing by necessity.

Study findings showed effectiveness of pregabalin in the treatment of neuropathic refractory pain. It was effective in patients with refractory pain to gabapentin, tricyclic antidepressants and third-line therapy. Effectiveness of pregabalin was similar in patients, receiving and non-receiving concomitant gabapentin. In 1/3 of patients pain was relieved by >50% and it was stable during 17 month-observation period. After 17 month – duration treatment with pregabalin proportion of patients with severe pain significantly reduced (from 63%  to 23%).

Biliary obstruction with hypertension and infection is the main cause of liver cholangiogenic abscesses (81.2%). Development of biliary hypertension in the patients with trans-hepatic or other type drainages (30) due to various reasons indicates the presence of stricture or tumor relapses (83.3%) and is the main cause of cholangiogenic liver abscesses.

Also, frequent episodes of cholangitis and bacteriocholia in the small group of patients (13, 16.7%) with no biliary obstruction and hypertension (during bilio-digestive anastomoses and the damage of Fater papilla sphincter apparatus) are the etiologic factor for cholangiogenic liver abscesses.

Objective of the research was to study age-related changes in Na+ and K+ ions of erythrocytes and their influence on changes in diameter. Studies have been done on three age groups: newborns (umbilical cord blood), adults (22-30 years old) and elderly (75-89 years old). Base on the results, we can assume that content of K+ is less in the erythrocytes of newborns, then among elderly people. This  causes erythrocytes hydration, an increase in size, and diameter of the erythrocytes of newborn becomes greater. Among elderly, diameter of the erythrocytes remains almost unchanged. But content of K+ in the erythrocytes of elderly increases and signifies their high-resourcefulness. The content of  Na+ ions remains unchanged among newborns, while increase among the elderly in comparison with adults.

ACUTE HEMORRHAGIC  EDEMA OF INFANCY (AHEI) IN PEDIATRIC PRACTICE

Acute hemorrhagic edema of infancy is a benign small vessel vasculitis that is characterized by edematous urticarial plaques that progress to purpuric plaques that often have a targetoid appearance. AHEI is most notable for its dramatic appearance and rapidity of onset within 24 to 48 hours. It is characterized by rash, nonpitting edema, and fever in an otherwise well-appearing child.

Clinical case of patient 16 mo, admitted to TMSU G.Zhvania Pediatric Academic Clinic, presented with  large, swollen blue-black lesion on his left cheek and new scattered bluish lesions on his thighs. The child had been diagnosed with a viral upper respiratory tract infection.  Diagnosed with AHEI. Treatment- IV infusion, H blockers, dexametazon. His inpatient course was notable for the changing nature of his skin findings and stable clinical status.

The scientific and technical progress in medicine and pharmacy does not reduce the use of herbal and animal origin natural medicinal products of ancient time, which are also well-known in our time, such as muds, clays, medicinal waters, and so on. Despite the fact that these medicinal productshave long been known for people, their study has begun only in the 19th century. Nowadays, scientists of almost all countries have come to the following well-defined conclusion: natural methods of treatment in many cases not only replace the synthetic medicinal products by their effectiveness, but with a number of indicators even exceed them, moreover, they do not have serious side effects.

Peloids are widespread on the territory of Georgia and hold a special place among the fossil deposits of the country. Adjara is one of the regions of Georgia, rich in peloids resources, which can be found on the seacoasts, as well as in the mountains. These resources have been known to the local population since the ancient times and have been widely used for therapeutic and preventive purposes.

It is well-known that medicinal muds have bactericidal, bacteriostatic, anti-inflammatory and antioxidant actions.The bioavailability of peloidsbiologically active compounds from ready-made drug forms have been also established.

The aim of our research was to study theanti-inflammatory activity of Ardagani Lake sulphide-silt peloid and of hydrogel prepared on the basis of thepeloid.

The objects of the study were the sulphide-silt peloid of Ardagani lake located in the Kobuleti municipality of Adjara region.

The anti-inflammatory activity of the study objects has been studied using formalin induced rat hind paw edema model.

The pharmacological experiments on white rats have been established the expressed anti-inflammatory effect of the studied peloid sample and itscontaining hydrogel.

The carried studies will promote the further research of the natural resources of Adjara region and will give a powerful stimulus to the development of the region’s resort and tourism activities.

In order to prescribe anti-inflammatory medications, a pharmacist should provide a patient with the information as to for what purpose, in which quantity and how often he/she has to take the relevant medication. This is due to the frequency of occasions when patients wrongly take anti-inflammatory medications in the process self-treatment. Thus, studying these medications is a pressing issue.

The aim of the research is to study the peculiar aspects of the application of “Toradol” and conduct a market analyses thereof.

The research has shown that, while selecting medications, patients give priority to effectiveness; they think that a medication should be accessible without regard to its price.

As the majority of the patients surveyed unambiguously declare, it is necessary that the quality of medications be examined and the latter should not be supplied just based on the trustworthiness of a particular country.

SYSTEMIC VASCULITIS IN CHILDREN: CLASSIFICATION AND CURRENT

Vasculitis in children is one of the challenges  of pediatric rheumatology. Many attempts were made to classify the different types of vascular impairments. The SHARE initiative represents an important major therapeutic vasculitides. IgA vasculitis is the new term for Henoch-Schönlein purpura (HSP), and is the commonest systemic vasculitis in childhood. It is defined in the latest Chapel Hill nomenclature (2012) as: vasculitis with IgA1-dominant immune deposits affecting small vessels. Antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitides (AAV) - comprise granulomatosis with polyangiitis (GPA; previously referred to as Wegener’s granulomatosis), microscopic polyangiitis (MPA), eosinophilic granulomatous polyangiitis (EGPA; previously referred to as Churg-Strauss syndrome). Renal morbidity and mortality is a major concern in the AAV, hence therapy aimed at preservation of renal function is a recurring theme for the treatment of AAV in both adults and children.

Thyroid pathology is the main problem for Georgia and belongs to the regional pathology. In recent years, autoimmune and oncological forms of thyroid gland have been significantly increased, despite the long-running iodiproplasticity, which makes it clear to other important factors that play a role in the course of this disease. These factors can be attributed to various chemical substances as a deficiency in the organism as well as toxic substances Structogen increases in the environment.

The article discusses the data in literature on the anthropogenic ecoplotters that are responsible for the pathogenesis of the thyroid gland and the study of which are necessary to reduce the number of thyroid gland pathologies in Georgia.

SYSTEMIC CYTOKINE PROFILE IN SEVERE COMMUNITY AQUIRED PNEUMONIA IN CHILDREN

Community acquired pneumonia is one of the main causes of mortality in children under 5 years. The outcome of severe pneumonia is due to interaction between the pathogen and the immune response of the host. Thus, the pro- and antiinflammatory cytokine levels should be important for predicting outcomes of pneumonia. The main aim of our research was to determine the possible connection between blood levels of cytokine and severe pneumonia. The study involved patients with severe pneumonia, whose were in the critical medicine department of M.Iashvili’ Central Clinic during 2013-2014. The research was approved by the Ethics Committee of Tbilisi State Medical University. The circulating cytokines (IFN-gamma, TNF-alfa, IL-8, IL-10) were measured in the first and fifth day of hospitalization by ELISA. Patient samples (sputum, urine, nasal-pharyngeal swabs, plasma) were tested for identification of pneumonia etiological factor(bacterial or viral origin).

The study revealed that patients with severe pneumonia, the first day of hospitalization has very high cytokine levels (IL-8, IL-10, TNF-α, INF-γ) compared to patients with moderate pneumonia. 1 patient who had very high levels of IL-8was died. Kids with pneumonia caused by bacteria has high levels of IL-8, IL-10, TNF-α, while in children with the pneumonia of viral and/or unknown etiology only the INF-γ was increased. Thus, the profile of systemic cytokines can be used for the development of pneumonia clinical severity and complications. However, before to the routine use of cytokines as a predictor of the disease, further research is required.

In work in a comparative aspect, the results of a social survey conducted among women aged 18 and older in Ukraine and Georgia on the same type of methodology are presented.

Preliminary analysis of the data made it possible to identify both the general trend and individual features. The latter manifested itself in the age structure, which was younger among the respondents of Georgia and a greater degree of manifestation.

In addition, they are less likely than women in Ukraine to contact doctors (49.6% versus 73.9%), while among them only 10.2% do not remember the time of treatment, whereas in Ukraine – 28.0% and statistically more those who are satisfied with the results of treatment. The worse quality of life is more often indicated by the respondents up to 40 years (61.2% versus 56.4% aged ≥60 years) and, on the contrary, in Ukraine – 44.7% and 58.9% - to 40 years and after 60, respectively.

The results of the study confirm the severity of the problem of urinary tract disorders in women, which acquires special significance in connection with the practical lack of a real understanding of the prevalence of pathology. There appear primary questions that require immediate resolution. First of all, they should include the epidemiological aspect, as well as carrying out large-scale educational work among the population aimed at familiarizing them with prevention, the need for early medical aid, medical treatment options. In addition to the tasks of practical urology, they are no less serious in the scientific plan and require the development of new improved methods of differential diagnosis and effective adequate treatment.
