HIRSUTISM STRUCTURAL AND CLINICAL-LABORATORY PECULIARITIES

IN GEORGIAN ADOLESCENTS

It was first established in Georgian adolescents with polycystic ovary syndrome (PCOS), idiopathic hirsutism (IH) and non-classic congenital adrenal hyperplasia (NCAH) that upper lip, thigh, chest and low abdomen areas are the main sites of excessive hair growth. 27.20% of all investigated patients with hirsutism had PCOS, 9.20% - IH, 1.92% - NCAH. From accompanied symptoms significantly prevailed acne, excitability and emotional lability (p<0.05), increased BMI (p<0.001, hyperinsulinemia/insulin resistance and dyslipidemia in all groups. Sleep disorders, rare food intake (<3) and/or breakfast skipping, carbohydrate-rich/low protein diet significantly often occur in adolescents with hirsutism which may causes the development of clinical and laboratory signs of metabolic syndrome.

Aim and objects was to study peculiarities and occupational particularities of professional for pharmacy faculty students’ in Georgia. The study was quantitative investigation by using survey (Questionnaire). Surveys was for pharmacy faculty student, 319pharmacy faculty students were interviewed in Georgia. Pharmacy faculty students’ 32.9% were from 17 to 20 years, pharmacy faculty students’ 64.9% were from 21 to 25 years, pharmacy faculty students’ 1.6% were From 26 to 30 years, pharmacy faculty students’ 0.6% were from 31 to 35 years. The main age of respondent pharmacy faculty students varies from 17 to 25 years. By the course pharmacy faculty students’ 25.4% were on Bachelor I Course, pharmacy faculty students’ 20.1% were on Bachelor II Course, pharmacy faculty students’ 23.8% were on Bachelor III Course, pharmacy faculty students’ 30.7% were on Bachelor IV Course. The main courses of respondent pharmacy faculty students varies from Bachelor I Course to Bachelor IV Course. On the question indicate your permanent place of residence? Pharmacy faculty students’ 64.6% were from Tbilisi, Pharmacy faculty students’ 15.4% were from Other city, Pharmacy faculty students’ 16% were from District-Region, Pharmacy faculty students’ 4.1% were from village. The majority of respondent pharmacy faculty students indicated permanent place of residence Tbilisi. The minorities were from other cities and region. On the question - How did you prepare for entrance exams? (You can indicate several answers)? Pharmacy faculty students’ 32% answer in the high school, pharmacy faculty students’ 22.3% answer  independently, pharmacy faculty students’ 12.2%  answer I have passed pre-study training  courses, pharmacy faculty students’ 64.3% answer  I studied with tutor. On the question - Are you satisfied with your professional (occupational) choice? Pharmacy faculty students’ 77.4% answer yes, I am satisfied with my professional choice, pharmacy faculty students’ 9.1% answer in general, I am satisfied, but I have some doubts about the preciseness of my professional (occupational) choice, pharmacy faculty students’ 8.5% answer I am partly satisfied with my professional choice, pharmacy faculty students’ 3.8% answer I am mostly disappointed with my professional choice, pharmacy faculty students’ 1.3% answer  I am not satisfied with my professional choice.

EPILEPTIC SEIZURES DURING ISCHEMIC STROKE AND ITS PREVALENCE

The goal of the research was to identify the frequency and type of epilepsy seizures during ischemic stroke in elderly patients. It also envisioned determining correlation between the types of the seizure and the time of its emergence. Specifying preventive measures of epileptic seizures after ischemic stroke was also the aim of the research. An epileptic manifestation often reveals during the ischemic stroke and in our research it constitutes 13%. Types of epilepsy seizures developed during ischemic stroke prevail a simple partial seizure rather than complex partial or generated, and the seizure occurs more often after 7 days after stroke exposure (in late period).

Correlation between the neurologic severity and categories of seizures revealed during ischemic stroke have not been seen through our studies; At the time of ischemic stroke, preventive treatment with anti-convolants reduces the risk of primary seizures as well as the frequency of repeated heart failure.

The data obtained  that hyperglycemia suppresses adaptive processes of angiogenesis, reduces the expression of  VEGF in endothelial cells; Reduces the direction of migration of endothelial cells and the ability to form blood vessels; whereas Vitamin C and E stabilize metabolic processes in the vascular system with its antioxidant effect.

105 patients with ischemic heart disease have been divided into 2 groups: with symptomatic and asymptomatic ST segment depression. Asymptomatic group had higher pain perception threshold estimated by skin warm up method than those with symptomatic ST depression, but general nociceptive hyposensitivity is not the unique cause which determines painless episodes of ischemia. Silent ischemia has been found also in some patients with low pain threshold. Symptomatic patients had more marked hemodynamic reactions during both tests than patients with painless ST depression.

In asymptomatic group dipyridamole test detected predominantly painless episodes of ST segment depression while in symptomatic patients - both symptomatic and asymptomatic episodes. It is possible that shift of blood to the sub endocardial layers can be one of the mechanisms of silent ischemia.

CORRELATION BETWEEN THE COMPLICATION OF INGUINAL HERNIA  SURGERY AND THE INITIAL CHARACTERISTICS OF THE PATIENT

The purpose of our studies is to determine the correlation between the peri- and postoperative complications and the initial characteristics of the patient and using different methods of surgery.

We observed 403 patients with inguinal hernia who underwent hernioplasty, including 98 with Liechtenstein, 74 with Gvenetadze and 231 with laparoscopy. The comparison between the groups was performed using Fisher tests and correlation analysis by of Spearmen, statistical analysis will be performed using SPSS 22.

The perioperative period following complications: intestinal damage, bleeding from the lower epigastric vessels, urinary retention, neuralgia, hematomas, postoperative wounds do not heal for a long time, in addition to patients who are operated on the Liechtenstein method, much more than the wound does not heal for a long time, pain, feeling of heaviness (p<0.05). Bleeding from the lower epigastric blood vessels and urinary retention were observed only after laparoscopic surgery. After Gvenetadze’s operation there was no intraoperative complication, migration of implants and relapses. Neuralgia was observed only in the case of Liechtenstein.

Correlation analysis showed that complications are associated not only with the methods of treatment, but also with the initial characteristics of the patient;

Conclusion: posttraumatic complications are associated with surgical methods, the type of inguinal hernia  and the initial characteristics of the patient.

SOME MOLECULAR FEATURES OF INNATE AND ADAPTIVE  IMMUNITIES

It is known that the development of immune response to pathogen involves a complex interaction of cells and molecules of innate and adaptive immune systems. Currently dendritic cells are considered as a connecting link between innate and adaptive immunity. Toll-like receptors function and expression are intensively investigated in norm and in pathology last decade, which are the most important members of the family of signaling receptors and play an important role in the activation of mechanisms of innate immunity. Infection is one of the major factors that influence in the expression of the toll-like receptors. The level of expression directly correlates with the severity of the process. In some cases Toll-like receptors allows as early markers of infection. Consider the role of the Toll-like receptors in the development of normal immune response, couldn’t exclude the various defects in signal transmission systems and in the receptors structure in patients with inflammatory diseases. That is why defining the role with of Toll-like receptors in various diseases is advisable and in the future will allow to affect the effectiveness of the therapy.

EXPRESSION OF CD34  IN RATS’  PANCREAS ISLETS IN ALLOXAN DIABETES

Pancreas islets while alloxan diabetes were studied  by morphological research methods.  Based on the conducted investigation it was stated that interdependent changes take place in â-cells and capillary endothelium of rats’ pancreas islets.

At an average severity of experimental diabetes parallel to the changes in the islets, activity of compensatory-adaptive processes are observed, proved by the increase of CD34 expression. It indicates to the active processes of migration, proliferation and differentiation of the endothelium and pancreatic duct cells of the islets.

SENSITIVITY OF BACTERIOPHAGES SPECIFIC TO B.ANTHRACIS TO DIFFERENT ENVIRONMENTAL FACTORS

Four bacteriophages active to B.anthracis  - Ba GD, Ba PAT, BaKu1 and Ba INSL isolated   from environmental sources in Georgia, have been tested under different pH, temperature and UV exposure, desiccation, stability in the different media. The studied phages  were characterized by sufficient resistance to different environmental factors. They revealed high stability in the three media, retained activity in intermediate interval of pH (pH4-9) and were resistance towards UV radiation.

TREATMENT APPROACHES FOR COMPLICATED BONE FRACTURES WITH A SPECIALLY CONSTRUCTED EXTERNAL FIXATION APPARATUS

Back in 1998 we came up with basic model of external fixation apparatus in order to eradicate the flaws of external fixation practices.  Our bone fixation apparatus and methodology have been used in more than 1500 patients treated in clinic- “Traumatological hospital” during 2002-2017 years. In most cases (up to 95%) this method was definitive means of treatment and didn’t need combining other techniques. In all cases the result was satisfactory, which is demonstrated by speeding up a consolidation process, reducing a length of stay in the hospital, achieving early mobilization and social adaptation. Considering all the above, we recommend using our fixation apparatus as a reliable and tested method for treating complicated fractures and receiving successful results.

It was discovered as in literary data, so in our own research, that Mildronat (Meldonium), as cytoprotector and general tonic medicine that improves metabolic processes in cells, is advisable to use in treatment of those pathological processes and diseases which are characterized by deficit in oxygen and substrate supply and high probability of developing ischemic damage.

INTRAOPERATIVE PROPHYLAXIS OF THROMBO- AND FATTY EMBOLISM IN CEMENT HIP ARTHROPLASTY

Thromboembolism and fat embolism are intraoperative complications associated with the use of bone cement during hip replacement surgery, especially during fixation of a femoral component or placement of cement inside the bone canal. Severe, as well as a moderate and mild cases of embolism are associated with either thrombotic masses or fat droplets. Causes may include small droplets of cement as well. This condition might be called a cement embolism. Reasons for embolism can be considered to be a specificity of a venous system of the bone structure and increase of internal pressure during cement placement.

Therefore for decreasing pressure inside a bone canal we use this simple procedure: we start drilling cortical layer of a femur bone and 1 cm proximally from a bone plug placed inside the canal. Procedure is carried out before placing cement. Mentioned simple method significantly decreases development of severe, as well as mild cases of embolism. We have studied 195 cases. Development and manifestations of embolism equal to 0.1%, applies to only mild embolism. Therefore we recommend this simple procedure that would significantly decrease a lethal outcome.

A STUDY OF THE EFFICACY AND SAFETY OF INFLUENZA VACCINE

Some criteria for studying the efficacy and safety of influenza vaccine presented by the WHO are considered.

The results of the efficacy and safety of vaccine produced using natural zeolites are presented.

It was found that the oral administration of the influenza vaccine with zeolite is more effective than the intramuscular route of administration. It is concluded, that produced vaccine is effective and safe.

ASSESSMENT OF SUBLINGUAL TYPE OF IMMUNOTHERAPY IN PATIENTS WITH MILD AND MODERATE PERSISTENT BRONCHIAL ASTHMA

The effectiveness of sublingual immunotherapy in treating of patients with mild and persistent bronchial asthma was assessed on the basis of Ismigen(polyvalent lysate of bacterial origin)administration. Peroral immunization of patients with polyvalent lysate during 3 monts resulted in significant improvement of their clinical symptoms, reduction in need of long-acting beta-agonists (LABA) and dosageof  inhaled steroid, also with marked improvement of spirometric values. After ending of treatment these effects were retained at least for several months.
