J. Experimental and Clinical Medicine, 2017, 4, pp.9-22

The circulatory state of the cardiovascular system (CVS) in pregnant women with physiological pregnancy, with perinatal pathology and in the same age group of nonpregnant ones is considered on the basis of the anthropophysiological approach, which is aimed at bearing pregnancy primarily under typical for humans upright walking conditions. An algorithm for systemic diagnostics of the CVS circulatory state, on the basis of which a new principle of hemodynamic identification of circulatory syndromes of heart failure (HF) is presented, has been considered. The data of the trimester dynamics of the circulatory state of the CVS and the manifestation of the basic syndromes of HF (by perfusion, stagnation) on the left and right heart ventricles, which are determined as a predictor of possible violations of hemodynamic support of pregnancy are shown and discussed.

MORBIDITY TENDENCY OF TELAVI, GURJAANI AND LAGODEKHI

MUNICIPALITY POPULATION

Iv.Javakhishvili Tbilisi State University, Georgia

J. Experimental and Clinical Medicine, 2017, 4, pp.25-31

The goal of our research is studying of the late effects of the lead minor, non-toxic doses exposure. We have studied the indicators of spread diseases during 2000-2014 years in Georgia, Telavi, Gurjaani and Lagodekhi, according to the public information of the National Center of Disease Control of the Ministry of Labor, Health and Social Affairs.

After analyzing the materials has been revealed, malignant tumors and ischemic heart disease rates statistically reliable surplus in Telavi and Gurjaani Municipalities, compared with the morbidity average indicators of Georgia. We may suppose that this is the late effects of lead exposure. In addition, it is notable that the statistical analysis of malignant tumors and ischemic heart disease rates has revealed statistically reliable paucity of the same rates in Lagodekhi, than in Telavi and Gurjaani rates. The above mentioned may be considered as one of the arguments for supposition made by us.

Based on mentioned we decided to analyze the records of ontological and cardiology patients’ medical records in Telavi and Gurjaani Municipalities’ population and perform epidemiological research through special questionnaires.

After our inquiries no reliable intraregional difference in food ration and giving priority to any food was detected. Most of the interviewed respondents get natural pectin and less get organic sulfides, that may determine the excessive load of the most of the interviewed respondents with lead ions and development of the existed pathological processes as a result.

Presumably, the regularities after analyzing the collected materials and further high-technology researches may enable new comprehension of various diseases genesis, prevention and treatment tactics.

FEATURES OF STROMA-PARENCHYMAL INDEX OF CHORION VILLI IN SPONTANEOUS ABORTION

Tbilisi State Medical University, Georgia

J. Experimental and Clinical Medicine, 2017, 4, pp.56-61

The aim of our study was to identify objective criteria for assessing changes in chorion villi  and other constituent components of the placenta.

To assess changes in chorion villus, we introduced a stroma-parenchymal index, which implies the ratio of the total area of the villi (V1) to the area of the villi stroma (V2). (Stroma-parenchymal index V1 / V2).

According to the data obtained, the stroma-parenchymal index with a normal chorion villi is almost exactly one. This means that in the normal chorion villi, the stroma and parenchyma content is almost equal. The highest index of stroma-parenchymal index 4.4 is noted with villous chorion with fibrous stroma and variable for various lesions of villi. There was no significant difference in the stroma-parenchymal index between complete and partial mole, but they differ significantly from normal chorion villus.  In complete mole stroma-parenchymal index is 2.9 times greater than the stroma-parenchymal index of the normal chorion villi and at partial mole is 2.6 times greater.

Also important are the correlation of changes in the stroma-parenchymal index of the chorion villi during mole and hydrophilic changes in the villi. With complete mole the stroma-parenchymal index is 1.9 times less than the stroma-parenchymal index with hydrophilic changes in villi, and 1.6 times less with partial mole. In cases of acute and chronic inflammation of chorionic villi, no statistically significant differences were noted.

The stroma-parenchymal index of the chorion villi gives an opportunity for an adequate and objective assessment of the changes in the chorion villi. It can be used as an auxiliary objective criteria for diagnosis of trophoblastic diseases.

CORRELATION ANALYSES BETWEEN HERPES SIMPLEX VIRUS AND OTHER PATHOGENS CAUSING INFECTIONS IN FEMALES OF REPRODUCTIVE AGE

1G.Eliava Institute of Bacteriophages, Microbiology and Virology;2Bacteriophage Analytical Diagnostic Center ,,Diagnozi 90”;3Eliava Phage Therapy Center, Tbilisi, Georgia.

J. Experimental and Clinical Medicine, 2017, 4, pp.61-65

Diagnosis of the genital infections in females of the reproductive age is essential. Herpes Viruses are characterised by a high risk of reactivation and a fast progress of the infection.

Various microbiologic and immunologic tests allow doctors to study the microflora of the uterus and depending on obtained results to select optimal strategy of treatment.

The results of the study demonstrated that the co-infection of bacterial pathogens and Herpes Simplex Virus, increased the persistence of the virus and its transmission rate.

No clear distinction can be drawn between HSV-1 and HSV-2. Based on the immunological studies it was concluded that in contrast to acute infections, the incidence of chronic infections was much higher (67%).During the study of HSV infections, several patients were also diagnosed with Hepatitis C and HIV that was associated with other polymicrobial infections.

The study is not an alternative to the diagnostic system - TORCH. Despite the fact that this system includes identification of Herpes Simplex virus, our study indicates determination of other (O) infections too.

J. Experimental and Clinical Medicine, 2017, 4, pp.65-69

The effect of green tea leaf extracts on the intensity of apoptosis in intact and incubated under oxidative stress conditions Jurkat cells was studied.

Studies carried out by spectrophotometry (MTT test) and flow cytometry showed that C, and E and E+C vitamins reduce the viability of intact Jurkat cells, but exhibit cytoprotective activity in the case of incubation of cells under oxidative stress conditions.

The extract of green tealeaves, in particular green tea catechins, increases the proliferative activity of Jurkat cells incubated under oxidative stress, but does not affect the viability of intact cells. Green tea pectin exhibits a cytotoxic effect on intact Jurkat cells, but exhibits weak cytoprotective activity on cells incubated under oxidative stress conditions.

Ovarian borderline tumors represent a separate group of ovarian epithelial tumors, which are characterised by non-invasiveness, heterogeneity and uncertain malignancy potential. There are certain criteria for histopathological diagnosis of ovarian borderline tumors. However, the detection of invasiveness (microinvasiveness) is complicated in some cases.

The molecular and epigenetic markers of borderline ovarian tumors are under investigation. These novel markers will help to determine the full differential diagnosis and prognosis of these in the future.

Ovarian borderline tumors represent the heterogeneous group with various histology and molecular phenotype – so called diagnostic ’’grey zone’’. The determination of biologic behaviour and hence the implementation of relevant treatment-prophylactic means represent the subject of intense investigation in the future.

ABOUT DEMENTIA

Shota Rustaveli State University, Batumi, Georgia

J. Experimental and Clinical Medicine, 2017, 4, pp.75-77

Dementia is a general term used to describe a loss of memory that keeps getting worse - or is ‘progressive’. There can be many different causes, they will all involve loss of memory, but they have other symptoms which are different according to the cause. Dementia often starts off with just memory problems, but can also involve:  difficulty planning and carrying out day-to-day tasks, difficulty communicating  and changes in mood,   judgement or personality.

Aim of this article is to identify spread of dementia in 200  patients  who have not been diagnosed before . Research was done in ONCO center of Adjara during 2014-2015, involved 200 patients, female - 130, male - 70.  For diagnosis of dementia we studied anamnesis, physical data, psychic and cognitive development of patient, family members were questioned often because of the difficulties patients had to explain symptoms they experienced.

All patients had CT scan and evaluated bey Vavilov Method. Arterial Hypertension ,Discirculatory encephalopathy -171, Cancer-5 , Parkinson disease-10 , endocrine disorders-14 , family history of dementia -3 were noted in patients. From 200 patient examined 170 were diagnosed with dementia , Light dementia(10%). experienced memory deterioration Average quality dementia (55%), During severe dementia (35%).

Some risk factors for dementia, such as genetics and  age. Cannot be changed. but researchers continue to explore the impact of other risk factors on brain health and prevention of dementia.

RISK FACTORS AND ASSESSMENT OF THE TOTAL RISK OF CARDIOVASCULAR DISEASES AMONG ADJARA POPULATION

Caucasus University, Tbilisi; Shota Rustaveli State University, Batumi, Georgia

J. Experimental and Clinical Medicine, 2017, 4, pp.84-94

The prevention of heart attack and stroke through an integrated approach, taking into account the overall cardiovascular risk, is more cost-effective than treating only thresholds for selected risk factors and should be part of a basic package of services to ensure universal coverage of the health - Health care. To achieve this, strengthening of the basic components of the health system will be required, including the financing of medical services to provide access to basic health technologies and essential medicines for the treatment of NCDs.

It should strive to adequately control the main parameters of the modifiable risk factors - body mass index, blood pressure, lipid profile and blood glucose. At the same time, special attention needs to be paid to pharmacotherapeutic correction of the set of target values   in order to reduce the risk of developing cardiovascular complications.

It is necessary to increase the attention of specialized outpatient clinicians to identifying and reflecting information on modifiable risk factors for cardiovascular diseases in medical records.

It should strive to expand and optimize the administration of statin therapy with adequate therapeutic doses and longer courses of treatment.

It is necessary to use more widely oral oral anticoagulants according to indications, with mandatory proper monitoring of the hemostasis system.

When prescribing anti-anginal and antihypertensive prophylactic drugs, one should use non-threshold, and at least medial, doses.

More attention should be paid to non-pharmacological correction of risk factors, especially such as overweight and smoking. It is necessary to properly advise the patient about the rational diet and physical activity.

FUNCTIONAL MORPHOLOGY OF LIVER AND INTESTINE IN MICE UNDER THE ZEOLITE DIET

Tbilisi State Medical University, V. Bakhutashvili Institute of Medical Biotechnology, Georgia

J. Experimental and Clinical Medicine, 2017, 4, pp.94-97

The purpose of our work was to study pharmacodynamics and influence of the zeolite “diet” (5% food supplement of clinoptilolite) on functional morphology of liver and gastrointestinal tract of white mice.

The used dose of zeolites was non-toxic and lethal to intact mice, has not cause allergic reactions. Zeolite “diet” has been followed by the improvement of glycogen indices in hepatocytes (23,5% more, then in the control), and by the intensification of ion exchange in the small intestine (the amount of ATP was 73% higher). It can be concluded that intracellular regeneration in the liver and intestine is a morphological manifestation of cell activation.

THE STUDY OF  DIABETES-INDUCED MORPHOLOGICAL CHANGES IN PREGNANT MICE UTERUS

TSMU Institute  of Medical Biotechnology, Tbilisi, Georgia

*P. Shotadze Tbilisi Medical Academy, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2017, 4, pp.97-103

Perinatal mortality is one of the main complications of diabetic pregnancy. The aim of our study is to evaluate the role of IL-10 in diabetes induced pregnancy loss and investigation of  diabetes-induced morphological changes in pregnant mice uterus. To clarify this question, we performed the study in streptozotocin (STZ) treated FVB female mice. To investigate influence of IL-10 administration on pregnancy loss and morphological changes in diabetic uterus depending on severity of diabetes, mice were injected i.p. with 50 mg/kg interleukin-10.

Therefore, pregnancy rate and resorptions quantity were investigated on day 9 of pregnancy, when implantation period is completed and immunohistochemically were detected apoptotic nuclei and proliferation sites in uterus during pregnany. In conclusion, the data presented in this article suggest that IL-10 play a central role in the pathogenesis of diabetes- induced pregnancy loss. IL-10 act as a suppressor of diabetes-induced embryotoxic stimuli leading to the death of peri-implantation stage embryos. IL-10 reverses of diabetes-induced abortion in this stage by modulating morphological changes occurring in diabetic uterus.
