Correction non-modifiable risk-faktors in adults

J. Experimental and Clinical Medicine, 2011, 5, pp.8-11

The Science Academy of preventive medicine of Georgia

Identification of arterial hypertension, overweight, low physical activity, smoking, alcohol and salt over usage, psycho-emotional stress and drug addiction in the same populations. Stratification of examined populations to risk-groups (low, middle and high risk groups). Development and implementation of differentiated preventive intervention programs for middle and high risk groups.

Development of measures for healthy lifestyle principles promotion (development and distribution of educational literature taking into consideration specific features of the population, trainings with active participation of adults, information and involvement of mass media).

Dynamic supervision of the whole population (with examination of focus groups) and implementing preventive activities.

Assessment of regular dynamic supervision and active intervention significance for risk-factor correction (repetitive testing of population, examination to detect risk factor frequency).

Development of the model for youth education on healthy lifestyle principles and its recommendation for implementation in public health

Epidemiological survey for chronic biliary tract diseases

J. Experimental and Clinical Medicine, 2011, 5, pp.11-14

The  Academy of Science Preventive Medicine, Tbilisi, Georgia

The incidence  of the right  hypochondrium  syndrome was  studied  among  5038  population aged 20 to 59 years in Tbilisi. The  relationship  was  defined  between  this  syndrome  and  some  risk factors  of cholelithiasis. The term right hypochondrium syndrome (RHS) was  applied  with a purpose of specifying  a  trait marking certain diseases including different cholecystopathies. The RHS was detected  in 3,54 of men and  9,6% of  women. The RHS incidence correlated with the  following   risk factors: female sex, age (40-59 years), overweight, and upset lipid metabolism. The subjects  with  the RHS are to be subjected  to further diagnostic examinations and prophylactic and treatment    measures  aimed  against  obesity, upset  lipid  metabolism  and cholelithiasis.

HEART RATE VARIABILITY AND HISTOCHEMICAL STUDY OF BRAIN CORTEX AT IMMOBILIZATION STRESS

J. Experimental and Clinical Medicine, 2011, 5, pp.14-18

Armenian State Pedagogical University department human and animal physiology,Yerevan

Immobilization stress is the psycho-emotional stress (with weak physical and strong psychological components). The purpose of this research was the study of peculiarities of functional state in rats in the dynamics of post-immobilization stress. The comparative analysis of the results obtained in stressed rats through the methods of MA (mathematical analysis) VHR (variability of heart rate) detected significant changes in functional state of animals. During 7 days there was no tendency to return to the studied indicators to normal: even in this period the animals were in the state of stress reaction. Sustained psycho-emotional tension can favor the development of stressful diseases.

comparative analysis of efficiency of peeling in the treatment

of melasma

J. Experimental and Clinical Medicine, 2011, 5, pp.18-24

Tbilisi State Medical University, Medical Center “Marjani XXI”, Georgia

Melasma is a very common cutaneous disorder which affects all races, but there is a particular prominence among females during their late third decade of life. Many depigmenting agents and other therapies are used for treatment of melasma, in the form of monotherapy or combined therapy, but the results are often unsatisfactory. In the present trial, including 20 patients with epidermal type of melasma, was studied the tolerance and effectiveness of alfa-beta and retinol professional peeling of Holy Land Cosmetics in comparison with results of chemical peeling with Jessner solution. Evaluation of skin condition was done in accordance with MASI score before and after treatment. After a follow-up period of six months, the MASI score was found to be reduced in 73,2% in the case of peeling by Holy Land product  and  30,4% in the case of Jessner solution. The results of the trial gives us a possibility to recommend the above mentioned Holy Land product for successful treatment of epidermal type of melasma.

PAIN PREVENTION, CONTROL AND MANAGEMENT IN NEONATES

J. Experimental and Clinical Medicine, 2011, 5, pp.24-28

Children New Clinic, Tbilisi, Georgia

From 24 weeks post-conceptional age all neurotransmitters and receptors associated with pain modulation are present and responsive, thus fetus and newborn can feel pain. Pain can cause immediate adverse physiologic effects (tachycardia, blood pressure changes, hypoxemia, tachypnea, increased release of cathecholamins, steroids, glucagon) and long term behavioral changes.

For any potentially painful intervention in neonates pain assessment using scoring stools (NIPS,  CRIES, PIPP, N-PASS) and pain  management or analgesia should be considered. Guidelines for pain management are: prevention or minimization of painful procedures; analgesia before painful procedures; non-pharmacologic or pharmacologic interventions.

While differentiating the forms of the outcomes, the study found psychotic form of residual condition in 9.5%, the form with reduced positive symptoms in 35.8%, deficit form in 42.0% and outcome in the form of clinical recovery in 12.8% of the investigated cases. Different clinical types of hallucinatory-delusional disorders were observed among the patients with psychotic form of the residual condition. The study classified the patients into the hallucinatory (30.4% of the all psychotic conditions), hallucinatory paranoid (13%), hallucinatory paraphrenic (13%), paranoid (26.1%) and paraphrenic (17.5%) forms of the residual condition based on the predominance of the psychopathological syndromes.
