Humoral immunity was studied in 60 patients (aged10-60) with acquired tracheal stenosis. The study discovered humoral immunity dysfunction which was most pronounced in the patients who developed scarry stenosis after critical care for acute stenotic tracheobronchitis. It was detected that if during treatment a patient has allergy (atopy) requiring long-term nasotracheal intubation, he/she is at risk of scarry stenosis development.

Clinical manifestations of intestinal dysbiosis are not pronouncedly marked in all diseased persons. In most cases parameters of bacteriological researches don’t allow to define features of clinical course of the disease in patients with various intestinal dysbioses. The estimation of clinical importance of dysbiotic changes should be based first of all on clinical manifestations, and not just on the results of a research of intestinal microflora. The subsequent course of the disease in many respects depends on duly and authentic diagnosis. The basic clinical manifestations of intestinal dysbiosis are: disturbance of the general condition (intoxication, dehydration), body weight decrease, symptoms of affection of the mucous membranes of the gastroenteric tract, digestion disturbance in various departments of the gastroenteric tract.

THE INFLUENCE OF ARTERIAL HYPERTENSION ON THE PARAMETERS OF CLINICAL/METABOLIC CONDITION OF WOMEN WITH TYPE 2 DIABETES

The purpose of this work was to research correlation between HbA1c and other risk factors of diabetic vascular complications in women with type 2 diabetes in the presence or absence of arterial hypertension.

A representative group of 315 women with type 2 diabetes was observed. In the absence of arterial hypertension, HbA1c correlated with age of the patients (r=+0.37±0.133; P<0.05), diabetes duration (r=+0.61±0.113; P<0.001), DAP (r=+0.31±0.136; P<0.05) and Triglycerides (r=+0.29±0.140; P<0.05).

In the patients with arterial hypertension, HbA1c correlated with diabetes duration (r=+0.20±0.097; P<0.05), fasting glycemia (r=+0.38±0.093; and P<0.001), VLD Cholesterol (r=+0.39±0.093; P<0.001), Triglycerides (r=+0.36±0.094; P<0.001).

Hypertension angioencephalopathy (HE, registration category in IDC-10- 167,4) is more often reason of chronic ischemia of brain (CIB), it is heterogenesis clinical syndrome. In spite of considerable sum of research about  searching of HE many aspects of  cerebrovascular pathology at this moment the disease was not studied till the end [4,5],  in that number  violation of venous circulation [1,2]. That’s why it is expedient the complex searching of clinical, neurovisualization, neurophysiology indexes during more spreading form of CIB-preddement  hypertension encephalopathy (II stage) and elaboration of tactic of correction violation with further evaluation of effectiveness of treatment under control of cerebral hemodynamic indexes.

68 patients with II stage HE  were observed (27 men/41 women). The criteria of choose the was presence of arterial hypertension with clinical cerebrovascular pathology (hypertension retinopathy), focal symptomatic and forming clinical syndrome and syndromes (vestibuloatactic, pyramidal, akineticorigid and others), absence of expressed cognitive violation and accompanying diseases.

This therapy impressed well with reference to subjective symptomatic and objective indexes REG. the reducing of essentiality of clinical symptomatic and cognitive disorders generally impressed well to common raising of quality of life of patients.

Effect of long-TERM ADMINISTRATION OF perindopril combinED with bisoprolol IN patients with chronic heart failure of ishemic genesis

In prospective observation are involved 78 patients with chronic heart failure of I-IV functional class according to NYHA, who had in the past myocardial infarction. Out of them 23 patients (the basic group) during 12 months took perindopril (2-4 mg/day) in combination with bisoprolol (1.25-5.0 mg/day). 53 patients (control group) received domiciliary treatment from doctors. The patients were examined prior to the beginning of therapy and by the 6-th and 12-th months against the treatment background. In the patients who protractedly took perindopril in combination with bisoprolol, in comparison with the control group patients, was marked an authentic positive dynamics of systolic and diastolic parameters of the left ventricle, which was accompanied by the expressed improvement of the functional condition, quality of life and survival rate.

THE RESULTS OF TREATMENT OF PATIENTS WITH  THE ZYGOMATIC-ORBITAL COMPLEX FRACTURES

We used conservative treatment for patients with the zygomatic bone fractures without displacement. Reposition of fragments with the help of Limberg’s hook and with a further palpatory revision through the oral cavity was carried out in early posttraumatic terms for isolated fractures of the malar arch and bone with a slight displacement of fragments. Osteosynthesis with titanium miniplates was carried out in the patients with the zygomatic bone fracture and slight displacement of bone fragments, and with the zygomatic bone fracture at three points and simultaneous linear fracture of the bottom of orbit. During multi-fragmentary fractures, the bottom of orbit was formed with the help of a perforated grid. The treatment for patients with posttraumatic defects and deformations of the cheekbone-orbital complex consists of plasty for the bottom of orbit via autotransplant. Due to the algorithms we have developed, treatment for patients with the cheekbone-orbital complex fractures develops a greater opportunity of obtaining positive results.

The purpose of the present research is to study the efficiency of drug-induced therapy for adolescent girls with pubescence oligomenorrhea in comparson with traditional therapy.

Adolescent girls with pubescence oligomenorrhea were studied. The research results showed that a new approach to the treatment of oligomenorrhea is well tolerated and has an obvious effect. Due to these corrections much better results were achieved expressed in weight loss, hirsutism reduction, elimination of vegetative–vascular disorders, as well as rehabilitation of menstrual and reproductive functions.

RESULTS OF OSTEOSYNTHESIS FOR MULTIPLE FRACTURES OF LOWER LIMB BONES

Treatment of patients with multiple fractures is an actual problem of modern traumatology and orthopedy. We observed 22 patients with 4- or 3-segment bone fractures (5 of them – with a 4-segment injury and 17 - with a 3-segment trauma) from 1999 to 2006, in the Research Institute of Traumatology and Orthopedy. There were 79 fractures in all. All patients were treated via the external fixation apparatus. Ostesynthesis of the femoral bone was performed by the Janbakhishov pin apparatus. All the patients had their bones healed. Few complications were observed (1 case – arthrosis, 1 case – limitation of knee joints movement, 2 cases – various deformations of the femur, refractures were observed in 2 patients). The treatment results were excellent for 8 patients, moderate - for 10 patients, and poor - for 4 patients. Thus, osteosynthesis with the help of the apparatus for external fixation is the optimal treatment for patients with multiple trauma or multiple fractures of 3 and more segments.

Indicators, characterizing the labor-dependent spread of ischemic heart disease among the male refugees and forced migrants aged 20-59 and settled in Sumgait, were higher for the temporarily unemployed in comparison with others. The most widely spread indicators of separate forms of ischemic heart disease were noted in the 40-59 age group. Thus, the IHD epidemiological situation is quite tense among the population belonging to the above-mentioned age group (irrespective of labor character) and requires preventive measures.

COMBINATION OF HEREDITARY ENZYMOPENIC METHEMOGLOBINEMIA WITH HEMOGLOBINOPATHY

The article presents the results obtained after the research of patients with combined methemo­globinemia and b-thalassemia. There was investigated the blood of 46 homozygotic and 84 heterezygotic patients with b-thalassemia aged 6 months-41 years.

For diagnosing were used isoelectrofocusing of globin chains, spectrophoto­metric and electrophoretic methods for definition of HbA2, HbF, MtHb, and enzyme MtHb-R activity. The received results show that during combination of methemoglobinemia with the homozygotic form of b-thalassemia, the amount of MtHb increases and MtHb-R level decreases as compared to heterezygotic forms. Family-genealogic analysis of relatives confirmed the diagnosis of a combined form of methemoglobinemia and b-thalassemia.

In the given article, the essential role of fungus  and ticks allergens in an etiology of an bronchial asthma is indicated.

The most essential value in formation of bronchial asthma is the sensitization to aeroallergens (allergens of the domestic dust, contained in it ticks Dermatophagoides pteronyssinus, Dermatophagoides farinae, mould, epidermis of domestic animal, birds, cockroaches).

The successes achieved in the field of pharmacotherapy of allergic diseases recently, reliably allow to supervise a clinical condition of patients during many years. However the pharmacotherapy neither cures the patient completely, nor warns transition of disease in heavier forms.

In this connection allergenspecific immunotherapy is the most effective method of treatment of atopic diseases.

Influence on all basic pathogenic parts is advantage of specific immunotherapy, causing expressed clinical effect which consists in restriction of expanding spectrum of a sensitization and progressing of pathological process.

Cardiocerebrovascular complications in patients with essential hypertension in the process of a long-term antihypertensive therapy (the results of a twenty-year-LONG observation)

Frequency of occurrence of cardiocerebrovascular complications was analyzed in 138 male patients aged 30-59 with essential hypertension in the process of a long-term antihypertensive therapy during a twenty-year period. After a 2-4-week placebo period, the patients were randomized according to five stratified criteria and received a mono- or combined therapy with various antihypertensive medicines. It was determined that the death-rate was related to the complications of arterial hypertension in 11.6% as a whole in the patients who received a long-term continuous antihypertensive therapy. The death-rate depended both on the disease severity and duration, and on the patients’ (especially those who received β-adrenoblockers) compliance in the treatment.

We have received the data of the epidemiological investigations of the female population aged 20-59 years, settled in Baku, and of the forced migrants living in Sumgait. The object of investigations was the prevalence of carbohydrate metabolism. We applied the methods for carbohydrate metabolism registration recommended by WHO. The frequency of carbohydrate metabolism disturbances didn’t differ much in both populations. Registration frequency of type 2 diabetes mellitus (DM type 2) was for certain higher among the women of Baku. Disturbed glucose tolerance (DTG) was absent in the entire female population of forced migrants. Disturbed glycemia on an empty stomach was registered 2 times more often among the forced migrants. An authentic difference was absent in the coronary heart disease rates (CHD), and also in the rates of type 2 DM and disturbed glycemia on an empty stomach (DGES).

Ultrasound screening has detected kidney anomalies in 4200 children (aged 0-18 months). It was determined that developmental anomaly of the kidney amounts to 161.3% in children and adults. Small developmental defects were found in 71.1%, congenital defects – in 21.1%, combined defects – in 5.6%. Congenital defects of the kidney are more frequent in boys. Thus, it is very important to add ultrasound examination to the complex of examination methods.

It is known that optimum formulating of microbiosiones is directly connected with normal micro flora’s source.  Such source of micro flora is mother.

One of the main conditions to formulate microbisenos is spending first hours of newborns with their mothers.   This problem assumes special importance in fact, that  children taken out with Cesarean section; so this group of children differently from children delivered normally  because they do not contact with birth ways of the mother and there’s opportunities of the colonization by the lactobacilli and bifidoflora is limited. The purpose of the offered work is to learn of intestinal microbiocenosis  forming and spectrum of microb biology in the children taken out with cesarean operation.    The main group of 36 newborn taken out with cesarean operation. There are 44 children which was born in natural way as comparison group have been investigated.

After the probioticoteraphy  in the newborns the colonization characteristics listed below are discovered in the intestine:  The amount of bifido and lactobacterious was increased, the level of E coli was normalized and the frequency of detection decreased. Thus, the changes in the intestinal microflora of the newborn, delivered with. Cesarean operation, are more changeable~ there is delaying of the forming of themicrobiocenosis in comparison with newborns, delivered in normal way, and don’t  complete in neonatal phase, not reaching the optimal level  of bifido and lactobacterious that provides the protection function at  the end of the neonatal period is requested to provide efficient correction in this type of children.

Clinical supervision used at 117 patients with defects of soft fabrics of the top and bottom finitenesses.  At 79 patients connected with traumas of a shoulder, loktevoqo a joint, a forearm and a wrist. The methods investigated by us not free plastics soft fabrics opens big possibmlity for carrying out reconstructively – regenerative surgical operations on restoration of defects of soft fabrics of finitenesses.  Aiso reduces risk of infringement of a feed (meal) of a rag mobilized of donor a box, the special toolkit for carrying out of such operations is not required, and also does not demand special preparation of surgeons.

THE IMPACT OF DIFFERET SUTURE MATERIALS ON PHARYNGEAL WOUND HEALING AFTER LARYNGECTOMY IN PATIENTS WITH LARYNGEAL CANCER

In spite of considerable success in anesthesiology and development of surgical techniques the rate of local postoperative complications remains high (10-20%).

The goal of our investigation was to determine the sort of suture material providing adequate healing of neopharynx.

The rate of complications such as pharyngeal fistula and paryngostoma was 12,5% (in 9 from total 72 patients undergoing total laryngectomy). Pharyngeal fistula was determined in 7 patients in which paryngostoma was formed by capron sutures and in 2 patients by using “vicril”. A scrupulous analysis of performed research allows us to conclude that the more effective material used for suturing of neopharyngeal defect in patients undergoing laryngectomy is “vicril” (p<0,001).

PARTICULARITIES OF REMODELINC OF THE LEFT VENTRICLE AND PRINCIPLES FOR CHILDREN TRETMENT

In 90th years the illnesses with diphtheria was increased in former Soviet Union, also in Azerbaijan. The aim of our investigation was to determine of late result of diphtheritic carditis in children by analysis of central and intracardiac hemodynamic parameters and structural-geometric indicators in early (after 2-3 years) and late (9-10 years) catamnesis. We examined 35 children aged 3-15 years with diphtheritic carditis in early and 15 children in late catamnesis by EKQ and EchoKQ meth­ods. In early catamnesis only in 14,2% children EchoKQ indicators were in norm, so at that children after diphtheria adaptive remodelling of heart was developed. But at 85,8% children it was developed desadaptive remodelling which brought to hemodynamic changes in 2 variants: a)in 57,1% of children volume and size of left ventricle was increased, but pump function was decreased (FE-=40~56%), so at that children dilatation cardiomyopathy was developed; b)in 31,4% of children size and volume of left ventricle were decreased, but pump function was in norm, so at that children restrictive cardiomyopathy was developed.

In late catamnesis in 89% of children we observed changes in functional condition of heart: dilatation cardiomyopathy wasrevealed in 66,6%; restrictive cardiomyopaty – 22,2%.  There were arterial hypotension and rhythm disorders in that period in all chil­dren. So in great  part of children with diphtheritic carditis in early and late catamnesis the desadaptive remodelling, which brought to development of dilatation cardiomyopathy, restrictive cardiomiopathy  cardiosc. Lerosis and vessel insufficiency was observed.

In including carnitine xlorid, vitamin E into complex therapy of children with diphtheritic carditis the systolic a diastolic function of myocardium was improved it’s show that both process has common pathogenetic mechanism.

Introduction.Postoperative and posttraumatic neurosensory disorders often arias at defeat of branches of  a  trigeminal nerve.

The purpose With the purpose of studying of this pathology we surveyed and have lead treatment of 62 patients with postoperative and posttraumatic neurosensory disorders. Depending on spent treatment of patients were divided into the basic and control groups. In  complex treatment of patients of the basic group joined magnetic laser therapy.

Materials and Methods. Depending on  ethiological factors of patients were divided  on two subgroups: patients with  neurosensory desorders after a trauma 34 (55% ) and patients with neurosensory disorders after operation 28 (45%). With the purpose of diagnostics and definition of efficiency of treatment method of electrotrigeminodiagnostics.

Owing to studying parameters electrotrigeminodiagnostics, treatment at patients from both groups it was defined positive dynamics on all parameters. But at patients from the basic group the analysis of results has shown effiency of treatment.

 Results of research.  Thus, the analysis of the received results has shown positive efficiency ETD methods at patients with after postoperative and posttraumatic neurosensory disorders of maxillofacial area.

Results of microbiological monitoring of infectious complications in a surgical hospital are discussed. The greatest attention is given contra – epidemiological to actions at allocation of microorganisms with plural medicinal stability. Actions on interruption of epidemic process of a hospital infection were based on revealing and dissociation of sources of activators of an infection (patients and the personnel), ways of transfer of an infection (contact, aerogenic), a susceptible organism. Actions on preventive maintenance of epidemic process of a hospital infection are based on introduction of an amplified epidemiological mode at revealing the patients allocating stammer with epidemiological  with significant markers of polyresistency and at presence or threat of an epidemic situation, revealed on distribution polyresistant stammer.

RESULTS of COMBINation APPLICATION IF-INHIBITOR IVABRADINE WITH

b-BLOCKER METOPROLOL at the PATIENTS WITH EXERTION ANGINA

The article gives 8-week treatment of 45 patients with exertion angina FC II-III (mean age 54,5±3,2 years), who were carried out IF-inhibitor monotherapy ivabradine (1 group, n= 5) and combination of ivabradine with b-blocker metoprolol (Group 2, n=20). To achieve the result of  HR (50 beats per minute), average daily dose of ivabradine as monotherapy was 11,5±0,3 mg (Group 1), while average daily doses of drugs in group 2 were 8,5±0, 6 mg for ivabradine, and for metoprolol 50,2±1,2 mg, accordingly. Grade of antianginal effect was carried out using bicycling sample. The research results showed that monotherapy with ivabradine resulted in a proper antianginal and antiischemic effects, that contributes to the enhancement of all indicators of tolerance to physical activity (p<0.05).

However, combination of ivabradine with  metoprolol contributes to the achievement of therapeutic effect to a greater extent than monotherapy with ivabradine (p<0.05). In addition, this combination of drugs can reduce side effects of b-blocker to 85% of cases.

Rating algorithm of immunologic data and degree of immunomodulation for patients with urogenital ureaplasmosis against complex treatment

Data of immunologic indicators of 127 patients with an urogenital ureaplasmosis with term of observation from 1 till 5 years which have received treatment by antibiotics of a wide action spectrum, the immunomodulating factor and an antioxidant are presented. The rating algorithm of degree of immune distresses and immunomodulation drug Cycloferon in three exploratory bunches depending on treatment methods paid off. Researches have shown, that a method of the combined therapy of patients with urogenital ureaplasmosis antibiotics Ofloxacin and Roxithromicin, immunomodulating factor Cycloferon and an antioxidant tocopherol authentic activating impact on middlemolecular peptides, phagocytic indicator, immunoglobulins G and also Ò-, Ò-helper, B, 0-lymphocytes makes, with direct correlation between results of treatment.
