BLOOD AND LYMPH COAGULABILITY IN THE CONDITIONS OF SUPEROXIDE DISMUTASE APPLICATION DURING EXPERIMENTAL ACUTE PERITONITIS

The purpose of the present research was to study in a comparative aspect the influence of peritoneal exudate on blood and lymph coagulability during experimental acute diffuse peritonitis. The experiments were carried out under narcosis on 34 “Chinchilla” rabbits of both sexes weighting 2.0-3.5 kg. Acute diffuse peritonitis was modeled by the method of V.V. Usikova et al. All animals were divided into 2 groups: 17 animals in each one.

The results of our researches have shown that intraperitoneal introduction of peritoneal exudates, taken from the animals with a modeled reactive phase of acute diffuse peritonitis, leads to the activation of intravascular coagulation not of the blood only, but of the lymph as well with the subsequent occlusion of haemo- and lymphatic capillaries. That proves to be true also with the reduction of lymph outflow rate from the thoracic duct in the animals of the control group. And intravenous introduction of SOD along with common peritoneal exudate prevented intravascular activation of blood and lymph coagulation, which was favorably reflected on RLO from the thoracic duct.

Comparing our data with the literature about the interrelations of POL and blood and lymph coagulability, and also about the larger maintenance of POL products in peritoneal exudate, it is possible to conclude that intravenous introduction of SOD, enhansing antioxidative potential in the skilled groups of the animals, prevents intravascular activation of blood and lymph coagulation against improvement of drainage function of the lymphatic system.

On the basis of modern approaches to the issues of classification and treatment coordinated with the Program of the European Association of Pathologists, the investigations of patho- and morphogenesis of TG diseases were carried out in the regions of Georgia.

We have examined surgery material from the patients operated in Tbilisi within the period of 2002-2008. The basis for the work included also the results of the retrospective pathomorphological analysis of TG patterns from the regions of Western and Eastern Georgia (material included 2529 cases) within the period of 2005-2008. 

The examined patterns of MTG revealed the increase of thyrocyte proliferative activity and basal membrane fibrosis and sclerosis. 

One of the distinctive signs of NEG is a great frequency of secondary destructive changes in the parenchyma. It is noteworthy that thyrocytes, cytoplasma and nuclei slightly exceed in size those of the persons from the control group, but compared to the types of toxic goiter, they are lower.

The analysis of the thyroid gland surgical material from various regions of Georgia shows that nodular euthyroid goiter with dystrophic-destructive secondary changes in follicles and stroma, with morphological signs of colloid stagnation and hypothyreosis prevails in the patterns from the regions of Western Georgia. In the patterns of surgical material from the regions of Eastern Georgia, toxic types of goiter prevail in particular multinodular toxic form with a high mitotic activity of thyrocytes, intensive vascularization and signs of an autoimmune disease.

Any intoxication is accompanied by infringement of metabolism and condition of bone tissues, where an especially great amount of alkaline phosphatase (AP) is aggregated.  

The increase of AP activity during acute poisonings attests to a substantial growth of the pool of osteoblasts i.e. new bone formation.

Hyperactivity of bone AP-isoenzyme before treatment and its propensity to normalization after treatment is a good prognostic sign. The high level of bone AP-isoenzyme attests to the incompleteness of osteogenesis. A proper assessment of the functional condition of bone tissues in patients with acute poisonings and possibility of its elimination allow to lower the percentage of complications both in the nearest and remote periods.

Thus, ectotoxicosis leads to the affection of the oral cavity organs and teeth. It occurs against the background of general changes in the biochemical processes of the organism, causing infringement of the activity of the studied enzymes in the blood and saliva of patients.

The research objective was to determine progesterone and estradiol levels in the blood of pregnant women with dental caries and inflammatory periodontal diseases (IPD) in comparative aspect. As a result of the researches, it has been revealed that the levels of fetoplacental hormones in the group of pregnant women with intensive IPD were higher, than in the group of pregnant women with highly intensive dental caries, and in the III trimester the level of estradiol was statistically significantly higher, than in the I trimester.

Cytological investigations of the membrane smears received from the antrum and body of the stomach have advantages over histological investigation of endoscopic biopsy materials received from the main part of the stomach. As compared with histological investigation, it’s distinguished by its high self–descriptiveness in the evaluation of growth level in the primary diagnosis of helicobacter infection. Not only the antrum of Pyloric mucous membrane, but also the infections of stomach liquid are mentioned in 80% of pyloroduodenal ulcer disease. The high level of H. Pylori growth in the stomach antrum and in the liquid of Pyloric mucous membrane is also characteristics of them. The number of the recurring bleeding patients with the high level of H. Pylori growth in the stomach antrum and liquid of Pyloric mucous membrane is statistically higher than the number of patient with non-recurring bleeding.

PERIOPERATIVE PROPHYLAXIS AND INTENSIVE CARE FOR HEMODYNAMIC DISORDERS IN ELDERLY UROLOGIC PATIENTS

79 urologic patients were examined for the assessment of character and frequency of cardiovascular diseases. According to their age, patients were distributed into three groups: I group – middle-aged patients (51.4±1.11 years), II group – elderly patients (68.9±1.12 years) and III group – old patients (78.7±1.08 years). The study includes clinical assessment of systolic, diastolic and mean arterial pressure, and of the condition of central hemodynamic parameters by the doplersonocardiographic method. The outcome of the research showed that changes in the hemodynamic characteriscics of elderly patients depend on the age-related physiological reorganization of the cardiovascular system and on the presence of cardio-vascular diseases. The authors discuss the relationship between the frequency of cardiovascular diseases and disorders in central hemodynamic parameters, the basic principles of hemodynamic parameters management during preoperative, anaesthesia, and postoperative periods and the ways of prevention and intensive care for perioperative cardiovascular complications.

In women of reproductive age, drug administration may cause a suppression of the innate immune system which in its turn causes suppression of vaginal microbial biotype and an intense growth of colonies of pathogenic microorganisms. In this regard, the activation of a candida infection plays the leading role.  

The purpose of our study was to estimate the effectiveness of candida infection treatment by Zalain. For that, we have examined 30 women of reproductive age suffering from a recurrent candida infection. Our study shows that Zalain appeared to be most effective in the treatment of this disease.

The obtained data promote revealing of the nature of local pathology and help to purposeful target pathogenetic therapy on the same basis.

A study of clinical presentations in the stomatological status of patients with chronic pulmonary tuberculosis has been carried out. A typical symptomatic complex of changes of the oral cavity mucous membrane has been revealed in the form of ulcer in the tongue, palate and gum areas.

It was determined that pathogenetic stomatological aid is an essential part of the general theatment for tuberculosis.

The present work is devoted to the study of mutual relations of the integrated characteristics of EEG with vegetative parameters in 15- to 18-year-old healthy teenagers under the conditions of quiet and intense vigil, negative and positive moods. It is established that on the basis of research and comparison at the integrated level of EEG and vegetative parameters in the conditions of quiet and intense vigil, it is possible to draw a conclusion that in healthy young men, as a whole, is marked a coordinated interaction of cortical areas and vegetative nervous system, reflecting synergic activity of synchronizing and desynchronizing systems of ergo- and trophotropic supersegment mechanisms either in ascending or in descending directions; the prevalence of each of these systems is correlated to a concrete situation.

Due to the activation of the hunger center, emotional stimulation of rats increases, determining the resistance of short-term memory to the negative influence of electromagnetic fields of communication frequency in white Wistar rats. As for the black rats, since they are characterized by high natural emotionality, their exposure to communication-frequency electromagnetic fields has caused the prolongation of short-term memory retard reaction.

We have studied the results of surgical treatment for III degree perineal lacerations and anal sphincter incontinence. In the preoperative period all patients heve been examined completely. Simultaneously were revealed functional disturbances of the sphincter. After preoperative preparations, the type of surgical intervention was chosen and scar tissue was excised. Postoperative periods were improved. Finally, surgical treatment of the patients with the mentioned pathology was generally improved.

Permissibility of one-stage performance of several surgeries in case of various diseases has been discussed in recent 2o-25 years mainly owing to the successes of diagnostics, anesthesiology and reanimatology. Hence, urgent combined surgeries for diseases of organs of the hepatobiliary system and accompanying pathological processes in internal female genitals are justified and expedient, irrespective of primacy of this or that affection.

Not eliminated combined diseases or division of combined operations into separate stages can become later the reason for complicated course (cyst suppuration or punching, purulent-septic process in the tube - pyosalpinx, destructive changes in the appendix).

MorphOLOGICAL AND functional changes of placenta during

simple herpes

The purpose of the investigation was to evaluate structural and functional changes occurring in the placenta of women with pregnancy aggravated by simple herpes. 72 pregnant women with simple herpes were researched by mycroscopic, morphometric and histologic methods. 25 pregnant women’s placentas were investigated in the control group. The disease reactivation was observed in 23 women. Morphological properties of the placenta were learned by macroscopic, morphometric and histological methods. The levels of placental hormones in pregnant women’s blood were investigated by the immune method to evaluate lidocrin activities, changes, characteristic of the placenta in pregnancy dynamics. In 22 (30.6%) observations were determined dystrophic changes in the inflammatory process. The number of villi with CD increased to a considerable amount in 21 (29.2%) observations (control – 37%, the main group - 68%); the number of villi with CKM didn’t differ from the indicators of the control group. Children were born in heavy hypoxia as a result of destructive neñrobiotic process that gains the upper hand over compensatory reactions in the placenta in 22, 6% ( 8.3% ) of observations. The villi with CD composed about 53 %, the villi with CKM made up 54 %. The level of placental hormones in blood serum was appropriate to the indicators of the control group in 53 % of women with simple herpes. The levels of herpecons 32-47 % decreased in comparison with the control group. During the observation, children were born with the signs of hypertrophy (19-26.4%).

A comparative analysis of the efficacy of laparoscopic Nissen and Toupet fundoplication DURING gastroesophageal reflux disease

The aim of the study was to evaluate comparatively the efficacy of laparoscopic Nissen and Toupet fundoplication on patients with gastroesophageal reflux disease (GERD).

Material and methods.  40 patients (24 males and 16 females) have been operated on for GERD 2006 through 2008. The age of the patients varied from 20 to 67 years (mean 47). The mean duration of the disease was 87±5 months. All patients underwent esophagogastroduodenoscopy, 24-hour pH monitoring and esophagomanometry. All patients were operated laparoscopically.

Results. The duration of Toupet surgery was 80±5 minutes and that of Nissen surgery - 70±5 minutes. There were no serious intraoperative complications or conversion noted. Mean hospital stay included 2.8 days for Toupet and 3.2 days for Nissen procedures. 10 patients (25%) developed early dysphagia after the Nissen operation and none after the Toupet operation. 3 patients underwent balloon dilatation due to severe dysphagia after the Nissen procedure. 2 of them were later repeatedly operated for the balloon dilatation inefficacy. There was no mortality noted.

Conclusion. Both Nissen and Toupet operations are effective for GERD management. However, the Toupet procedure has some advantages like absence of dysphagia after the operation and early rehabilitation of patients.

We used the method of ”autobiological filling” for the dogs with artificially damaged mucous membrane of the stomach and duodenum. The received favorable morphological results, correlating with the dynamics of hematological parameters, testify to the suitability of autobiological filling for wounds with the purpose of reinforcement of hemostatic seams for conducting organoprotective operations in case of profuse ulcer gastroduodenal bleedings.

We have performed clinical, laboratory, and diagnostic tests for 40 patients with III degree old perineal lacerations. During clinical examinations were revealed delivery conditions and causes of perianeal lacerations; there were determined infections originated from concomitant diseases. Besides, sphicterometry and manometry were conducted to determine the anal sphincter function.

Thus, complex methods of examination determined the type and capacity of surgical intervention.

AGE-ASSOCIATED CHANGES OF TREG CELLS IN HEALTHY POPULATION

Aging is associated with a decline in immune status that increases the vulnerability of older individuals to infections, cancers, and other chronic diseases. Genetic, environmental, and lifestyle factors play important roles in the rate of change in the immune system and, thus, in the development of age-associated diseases. Studying immunity in humans is therefore essential for developing treatments to restore dysregulated immune responses and assure healthy aging. The aim of our study was evaluation the potential risk factors (lifestyle characteristics, tobacco, alcohol consumption, nutritional parameters etc.) on CD4+CD25+ Treg cells quantity in Georgian population throughout ageing. The results of our study showed that percentage of studied immune (CD3CD4, CD4Foxp3, CD3CD8) cells is the comparative with the ranges in European population. The results of our study showed that CD4FOXP3 and CD3CD8 cells are increased in aged individuals and height and blood pressure does not determine quantity of immune cells.
