SOME INDEXES OF PROINFLAMMATORY CYTOKINES IN PATIENTS WITH CHRONIC HEART FAILURE OF DIFFERENT AETIOLOGY AND NYHA FUNCTIONAL CLASS

Background: Heart failure (HF) is characterized by a decreased exercise capacity with early appearance of fatigue and dyspnea. In order to achieve further improvement in the management of patients with HF, additional monitoring methods are required.

Aims- This study investigated “ protein status’’ indicators and the levels of tumor necrosis factor (TNF)-a and interleukin (IL)-6 in patients with chronic heart failure.

Methods and results - 95 patients with chronic heart failure of ishemic (n=50) or non ishemic(n=45) aetiology were enrolled. In the Studt plasma concentrations of creatinine, protein, albumin, ferritin, bilirubin, iron, lipid profile, TNF-a, IL-6 were measured. Total lymphocyte count (immunocompetence-IS), LVEF, BMI- were detected. No differences were found between the patient groups divided by aetiology.

ANALYSIS OF IMMEDIATE AND REMOTE RESULTS OF MAXILLARY CANCER COMBINED TREATMENT

The study presents the analyses of immediate and remote results of combined treatment of 51 patients with maxillary cancer. Evaluation of clinical and pathological parameters of maxillary cancer ensures the determination of prognosis of this disease. Consideration of this issue for determination of treatment tactics allows a significant improvement of (according to preliminary results by 20-22%) the outcome. This is quite a high percentage, if take into account the fact that the study group consisted of the patients with advanced stage maxillary cancer.

INNER-GROUP RELATIONS AND SHARE-INDICATORS OF PSYCHODYNAMIC INDEXS AMONG ACCELERANT GEORGIAN MEN AND WOMEN

There were researched 100 Georgian accelerant 17-22 years old 45 women and 55 men. It was established that Georgian accelerant women are constitutional types having phlegmatic (28,89%) or choleric (28,89%) temperament and middle logic intellect (62,22%); according to the character they are extroverts (53,33%) with weak excitement (55,56%), weak dynamics and strong statics (55,56%). Georgian accelerant men are constitutional types having phlegmatic temperament (45,55%) and middle mathematical intellect (67,36%); according to the character they are introverts (54,45%) and according to mood – dynamics (56,36%).

On the basis of the conducted research it was established that inner-group relations between temperament and character forms of accelerant women is in the limits of 0.2 – 0.3; and the interrelation between temperament and mood types is in the limits of 0.3 – 0.4.

The interrelation between temperament and character forms of Georgian accelerant men is in the limits of 0.3 – 0.4; relation between temperament and intellect subtypes – 0.2 – 0.3; and relation between temperament and mood types is equal to 0.2 – 0.3, i.e. comparatively low, than among accelerant women.

THE REACTION OF  BIOLOGICAL AMINES AND SEX HORMONES TO VIBRATION PATHOLOGY AND AFTER  CORRECTION OF THIS PATHOLOGY WITH OXYGEN

The research was held on two groups of 40 grown-up white male rats weighting180-200 gr.

The first group – the vibroseance for 20 grown-up male rats lasted for 1 hour every day during period of 2 months (with parameters: frequency  50 hz, amplitude 3mm). The modulation was performed in the kind of general vibration.

The second group-20 grown-up male rats against the background of vibratory stress (vibration  parameters were the same as in the first case) every other day were made hypodermic injections of liquid oxygen (Manufactured by  “AQUAGEN” international, IIVC. Oxygen Supplement).

After the tests finished had been we carried out decapitation  of the animals in the conditions of general narcosis and by means of the high pressure chromatography method in the hypothalamus. Concentration alterations of amines (dophamine, serotonine and norepineprine) were defined, and by immunoferment micro analysis of the blood plasma the concentration of gonadotrophin hormones  (Follicle stimulating- FSH and Luteinizing hormone- LH) and sexual steroids (testosterone T, estradiol E2) was determined.

For the control of the mentioned groups 10 grown-up male rats were taken, that were made all the mentioned researches.

The research showed that in both groups the increase of serotonine in the hypothalamus took place. in the first case its concentration increased up to 147% and in the second one to 135%. As the rate of norepineprine in the first  group was increased by 41%, against the background of oxygen injections its concentration reduced to 80%. It is especially worth mentioning that as in the first case the concentration of dopamine was too reduced to 14%, in the second case it made 80%.

In the both cases almost the same reduction of quantitative alterations of sexual hormones took place. although it should be mentioned that against the background of  oxygen injections the rate of LH slightly increased and a little exceeded the low limit of the norm.

CLINICAL AND EPIDEMIOLOGICAL PECULIARITIES OF  MEASLES INFECTION

In 2004, during the outbreak of measles infection in Georgia, 392 patients admitted to the Scientific-Practical  Centre of  Infectious Pathology,  AIDS and Clinical   Immunology   with  the diagnosis of measles have been subjected to a retrospective study. Out of  6847 cases of measles infection  having  place in  the whole  Georgia  1313  ones  were registered  in  Tbilisi. The Greatest number of  cases was noted  within  the period April-June and  the peak of  the disease was in  May. The  average  age of the patients  was  17 years. The  indices of  the unvaccinated  prevailed. During  measles infection in  Georgia, five patients died. Among the patients investigated at our clinic, lethal outcome  was fixed only in one  case – gravity and outcome of  the  disease proceeded  from  the complications  of  the  main disease, which  in it’s  turn  was  stipulated   by peculiar  aggravated  background  of  the  patient’s state. Among the most of the patients grave and atypical current of the disease was mainly caused on one side by the absence or disorderliness of vaccination against measles and on the other side by late hospitalization.

ULTRASOUND DIAGNOSTICS AND TREATMENT OF SIMPLE OVARIAN CYSTS

48 females of the reproductive age being observable for the menstrual cycle disorders and pain of various intensity in the lower abdominal region have been diagnosed for having simple cysts: follicular cyst in 32 patients, and corpus luteum ovarian cyst in 16 ones. All the patents underwent hormonal therapy with the monophasic oral contraceptives during 3 menstrual cycles.

As a result of the hormonal therapy, 23 patients with the follicular cyst have been recovered, while in all the 16 cases of the corpus luteum cyst the hormonal therapy has yielded no positive results, and these patients underwent laparoscopic treatment.

Dynamic ultrasound makes it possible to identify the conservative treatment terms and, where necessary, to timely transfer the patient to the laparoscopic therapy.

THE USE OF THE PREPARATION “TRICHOMICOCID” AND LAZEROTHERAPY FOR THE TREATMENT OF VAGINAL INFECTIONS

This article deals with a new pathogenetically documented therapeutical method in women with vaginal Trichomoniasis and Trichomono-candidosis infections: the intravaginal and per rectum administration of a new biologically active herbal medication “Tricjomicocide” suppositories (created in our laboratory). Adjuvant lazer therapy was also used. 127 women with urogenital Trichomoniasis and Trichomono-candidosis infection were selected, who applied for treatment to the Consulting-Diagnostic Center, Acad. K. Chachava Research Institute of  Perinatal Medicine, Obstetrics and Gynecology. 65 of them composed the main group (treated according to our treatment schedule) and 62 entered the control group (treated according to the conventional regimen). Curability rates in the main group made up 95,4% while those in the control group were 93,3%.

The term “Aggressive Atherosclerosis” was introduced by academician Nodar Kipshidze  and means malignant atherosclerosis caused by destabilization of “unstable achromatize plaque” that greatly depends on the inflammatory process taking place in it, and is predisposed to the rupture and development of local thrombosis. The aim of the work is to study: 1) relationship between the inflammation markers  and lipid spectrum levels in Helicobacter pylori (HP) and  Chlamydia pneumonia(CP) infected patients  with CHD, 2) the role of infection in the exacerbation of atherosclerotic process in  infected  CHD patients, 3) statins and infection (HP and CP) affecting drugs influence on the aggravation markers and aggressive atherosclerosis process. Materials and Methods: We selected 58 infected (HP  and CP) patients with CHD and  (proven by means of myocardial infarction documented in hospital and by ECG and veloergometere tests). Blood samples were collected for  each person after 12 hour fasting.

Non-hemolised serum underwent   the following tests: Total Cholesterol (TC), Tryglicerides (TG), High Density Lipoproteins Cholesterol (HDL-C). Fibrinogen in plasma was performed by Rutbergh  method, CRP - by immunoferment analysis. Anti CP IgG - by ELISA method. Statistical Analysis -continuous variables are presented as mean (M) + standard deviation (SD). Student’s T test for data was used to compare values within subject groups; Statistical significance was defined as P < 0.05. Correlation (r) were tested by Pearson  correlation.

Presented data shows: for stabilization athermanous plaques and treatment of Infected CHD patients, with eradication of infection, it’s necessary to treat with statins, which have an anti-inflammatory effect.

FLAP PREFABRICATION

An experimental study takes place on the basis of the Reconstructive, Plastic and Vascular  Microsurgery laboratory, at the Georgian Medical Academy.

The present study was designed to investigate the possibilities of neovascularization  after rotation of arteriovenous pedicle from donor to recipient site(flap prefabrication).

90 white Wistar rats weighting 150 to 300 g were used in this study. The rats were divided into 2 - experimental (70) and control groups(20). The experimental group was divided into 3 subgroups. I. group - Arteriovenous vascularized flap formation, II group – osteocutaneous flap prefabrication, III. group  - composite flap formation with the use of carbon. The control group was divided into 2 groups: 1. The flap morphological study and 2. Skin tunneling and morphological study. At different time intervals after prefabrication  (2week, 1, 2, 3, 4, 6, 8, 10 month) the flap was  studied morphologically and angiographycally. In experimental group the results showed a progressive degree of neovascularization that correspond, to the increasing length of time while the pedicle was implanted in the flaps. In prefabricated flaps the number of newly formed vessels rather increased of compared with the control group. The bone component was observed to be viable. There was no necrosis in the epidermis and dermis. Angiograms of all the groups showed potency of vascular pedicles.   

In case of donor site deficiency, vascularized composite flap formation is one of the main problem in plastic and reconstructive surgery.  Further study of flap prefabrication will improve the results of plastic and reconstructive operations.

EPIDEMIOLOGY OF TRICHOMYCOSIS IN ADJARIA

Dermatophytosis of thehairy part of head is one of the most commonmycoses in thexwhole world. Theigeographicdistribution is not the sameeverywhere. In some countries microsporia takes the leading place among them. At the same time, in some republics of  the former Soviet  Union trichophytosis makes the majority of fungal diseases with the damage of hairy part of head. Antroponosic trichophitosis prevails in some of them, zooatroponosic trichophytosis does in the others.

In Adjaria the struggle with dermatophytosis is constantly carried out. In 70 of XX century favus was practically liquidated. Trichophytosis used to take the first place by frequency of occurrence, antroponosic trichopytosis prevailed among dermatophytoses (pathogene – T.volaceum), whereas microsporia, brought about by M.ferrugineum was taking the second place. Trichopytosis and microsporia brought about by zoophilic fungi was seldom monitored. For the last 20 years the fungal flora of these dermatophytosessharply changed. Thesignificance of zooantroponosic trychophytosis inscreased (pathogene – T. mentagrophytes var. gypseum). Antroponosic trychophitosis is seldom monitored, antroponosic microsporia is almost liquidated. In recent years microsporia is brought about by fungus M. canis.

Sharp decrease of rate of sickness by dermatophytosis with predominant damage of hairy part of the head is caused by regular anti-epidemic meashures carried out in the country.

INFLUENCE OF PLASMAPHERESIS ON THE OUTCOME OF PEDIATRIC PATIENTS WITH SEVERE SEPSIS

Objective: Determination of risk of mortality risk in patients who received plasmpheresis with standard treatment for sepsis, in comparison to the patients receiving standard treatment alone.  Design: Prospective, randomized controlled clinical trial. Setting: Intensive Care Unite at Pediatric Clinic of Tbilisi State Medical University. Patients: Surgical pediatric patients with severe sepsis, septic shock and MODS. Interventions: 157 patients randomized to receive either standard sepsis treatment or an add-on treatment - plasmapheresis. Measurments: Primary endpoint was intrahospital survival. Mortality rates were detected in plasmapheresis and control groups and relative  mortality risk associated with plasmapheresis as calculated. Differences were considered statistically significant at P values less than 0.05. Results: Relative risk of mortality in the plasmapheresis group was 0.65 (95% CI 0.46-0.93) among the surgical pediatric patients with severe sepsis. This represents absolute risk reduction of 19.4% p=0.015. Subsequent investigation showed up a statistically significant influence of plasmapheresis on the outcome of patients in 1 month – 3 years old age group (absolute risk reduction of 51,8% p=0.003), while similar result didn’t reach significance in the other pediatric patients.  Conclusions: Plasmapheresis has an influence on mortality rate of surgical pediatric patients with severe sepsis, but the results are controversal in different age groups.

In  2000-2004 we studed 2392 petients. In etiological structure prevailed S. epidermidis (53,5%), T. vaginalis (42%), C. albicans (30%), S. aureus (15,6%).

In associations were mainly revealed  S. epidermidis+ C. albicans (30%), N. gonorrhea+ S. epidermidis (53,9%), S. aureus+ C. albicans (25%).Investigations     were carried out using modern standard technology .

The aim of the stidy was to investigate the ability of rat peripheral blood mononuclear cells to modulate proinflammatory reaction during experimental obesity. We studied the intensity of basal and LPS-inducible production of proinflammatory markers, reactive oxygen and nitrogen species in control and obese rats’ peripheril blood mononuclear cell culture. It was revealed that high calorie diet doesn’t influence the proinflammatory reaction of rat peripheral blood monocytes. So, the oxidative stress in the adipose tissue during obesity not coursed by diet intervention induced changes in monocytes function activity, but by their activation in the inflammatory area of the obesity tissue growth.

THE ROLE OF OXIDATIVE STRESS IN THE PATHOGENESIS OF ACUTE PANCREATITIS

Acute pancreatitis by its clinical multifamily and high lethality is an actual problem of modern medicine. It is revealed, that oxidative stress peays an important role in the pathogenesis of acute pancreatitis. The aim of our study was to establish the mechanisms of disorders in redox-homeostasis during acute pancreatitis in the experiment. 

During experimental pancreatitis (acute, infected, surgical), the activation of free radical processes, NO synthesis, accumulation of oxy- and lipoperoxides, inactivation of antioxidant ferments, and intensification of oxidative stress were revealed. It is significant that oxidative processes intensity is the highest in lungs, causing a high vulnerability of this tissue during acute pancreatitis.

Mitotic activity, amitosis and amount of degenerative nuclei were studied in epithelial cells from albino rats gingiva mucosal layer after experimental lesion by ligation. It was shown that mitotic activity is predominant way of tissues repair in operated gingival in experiments from 12 hours to 5 days.

COMPLEXIVE SURGICAL METHOD FOR THE TREATMENT OF MANDIBLE FRACTURE

For operative treatment of mandible fractures we undertook an attempt to develop a complexive method, using stable fixation of bone fragments by a titanium miniplate (NPP “LIM”), and filling the defect by osteoplastic material with long-term local antimicrobial action (Kolapan granules of the “Intermedapatit” company), biodissolving membrane Bio Gide (Geistlich Biomaterials) in combination with regenerative process stimulation carried out in the organism by the anabolic material retabolil (Gedeon Richter).

40 patients were under constant clinical observation in the process of treatment. Some complications were noted. A total period of disability lasted for 20 days. 4 weeks after the operation, no mobility of fracture fragments was detected, but fracture consolidation was quite satisfactory.

GREEN TEA CATECHINS – REGULATORS OF APOPTOSIS IN RATS SPLENOCITES

An excess production of free fatty acids in the immune (thymus, spleen) and nonadipose tissues (liver, smooth muscle cells) by the suppression of mitochondria respiratory chain promote the intensification of free radical oxidation processes. Dysfunction of mitochondria and chronic production of reactive oxygen species during obesity contribute to the development of oxidative stress, accumulation of lipid peroxidation products, and membrane damage, which play important role in the mechanisms of cell injury and induction of lipoapoptosis. The aim of our study was to establish the affectivity of green tea catechines during correction of oxidative stress - induced apoptosis in splenocytes cell culture.

As a result of our investigation, it was revealed that green tea catechins contribute to the normalization of mitochondria membrane potential Dyì in oxidative stress induced apoptosis in splenocytes. Such effect of catechines is caused by their corrective activity on the intensity of oxidative metabolism in cells and determines the antiapoptotic activity of these compounds.

MORPHO-FUNCTIONAL CHARACTERISCTICS OF PELIOSIS DURING INDEPENDENT CCl4 AND SIMULTANEOUS CCl4 / “KHVANCHKARA” WINE ADMINISTRATION

The goal of the study is to determine the nature of peliosis developed during cirrhosis and to study its morphofunctional changes during CCl4  administration together with the „Khvanchkara“ wine. The study has been performed on 40 adult white rats with liver cirrhosis modeled by subcutaneous injection of 0.1ml/100gr 40% CCl4. The injections were made twice a week. 20 white rats of the I series were injected with CCl4 only. 20 rats of the II series, while injected with the toxic substance, were administering per os 2ml/1kg wine „Khvanchkara“. The animals of both series were devided into 4 groups: the rats constituting the I group were sacrificed after 21 injections; the animals of the II group – after 21 injections and a one-week interval; the III group rats – after 40 injections; the rats of the IV group – after 40 injections and a two-week interval.

There have been used histological, electromicroscopic methods, and electromicroscopic cytochemistry method of staining for Ag-NOR protains. In the liver of the I series I group animals, there occur pelioses of  the regular geometric shape. These are strongly dilatated venous vessels (phlebectatic pelosis). In the III group animals, their number and mould increases. In most cases part of their wall is lined with endotheliocytes, and the areas of more or less length lack the endothelial cover. We couldn’t reveal parenchymal peliosis resulting from hemorrhages on the histological preparations. In the liver of the II series animals, there was not actually revealed any peliosis. There were noted only comparatively small-mould, single dilatated veins. Unlike that of the I series animals, their endothelial cover is intact, and there are not revealed any adjasent hemorrhages.

The preventive effect of the „Khvanchkara“ wine is conditioned by blocking the factors (ednotheliocytes alterations and desquamation, activity of stellate cells) causing veno-oclusion.

PREVALENCE OF ALLERGIC RHINITIS

The purpose of the work is to study epidemiology (prevalence) of allergic rhinitis in the population of school-age children in Tbilisi. Researches were conducted on the basis of questioning of random and representative groups of schoolage children by means of cross-section method of epidemiological research. At the first stage there were questioned 3000 children of 6-17 age, and at the second stage clinical-allergic examination was conducted with 256 randomized patients with the signs of rhinitis. In the population of Tbilisi children prevalence of allergic rhinitis was 13.2%, among which 98% were diagnosed primarily. Small number of cases was accompanied with conjunctivitis (2.4%).

Frequency of spreading of the signs of allergic rhinitis was surely higher in boys, as compared with girls. In the results of in vivo study of the allergens, there prevailed sensitization caused by domestic dust  - 67.2% of cases, and in 25% of cases it was caused by epidermal allergens of the cats and dogs (there were used prick-tests produced by ALK scherax) . Studies revealed high frequency of delayed diagnostics, light and medium condition, prevalence of intermissive allergic rhinitis.

UMBILICAL VEIN DRAINAGE FOR INTRAPORTAL CHEMOTHERAPY IN THE PATIENTS WITH COLORECTAL LIVER METASTASES

PURPOSE. The aim of our investigation was to perfect the umbilical vein recanalization and portal system drainage method, and to determine the possibility of conduction continuous   procedures (chemotherapy). 

PATIENTS AND METHODS. 141 patients with liver metastatic damage of the from colorectal cancer underwent umbilical vein recanalization and portal system drainage for intraportal chemotherapy.

RESULTS.  141 patients underwent 185 courses of 5-ftoruracil intraportal   chemotherapy. 3 courses were conducted for 42(29,7%) patients, 2 –for 54 (38,2%) ones. One course was conducted in all cases.

CONCLUSION. This procedure is not difficult for a skilled surgeon. At the appropriate management there is an exists possibility to conduct multiple procedures.

The number of people who have the secondary full adentia and need the complete removable plastic prosthesis permanently increases. The common problem in orthopedic dentistry is the fixation of the prosthesis of atrophied toothless mandibular. The obstacle for fixation of the mandibular prosthesis may be the place of fastening of tongue bridles and face muscles, also the topography of channels of salivary colic and mobility of tunica mucosa.

For the perfection of fixation of complete mandibular removable prosthesis we surgically performed the raising of atrophied alveolar ridges and then performed the prosthesis.

As a result of the objective research it was found out that the prosthetic bed expands by at least 55,6% and accordingly increases the fixation of complete removable prosthesis of mandibular.

The chewing  effectiveness of the new prosthesis increases two times in comparison with old prosthesis that was made before operation.

The tonus of chewing muscles stabilizes and in total terms of adaptation with complete removable plastic prosthesis reduces.

THE CLINICAL AND IMMUNOLOGICAL ASPECTS OF NECROTIZING ENTEROCOLITIS (NEC)

The aim of the study was to estimate diagnostic criteria, immune status  of NEC and ultrastructural investigation of peripheral blood formed elements – eosinophils and basophiles,  considering stages, severity, weight of a patient and tolerance of the newborn to operational intervention. 

During the last 10 years (1995-2005)  51 newborns with the diagnosis of NEC  were admitted to the department of  Pediatric Surgery of Prof. G. Zhvania Pediatric Clinic, from which 13 were with normal weight and term newborns, 19 – with birth weight 2500 – 2000 g, in 15 cases birth weight correlated between  2000-1500 g, and in other 5 cases the birth weight was 1500 -1300 g. 

In order to systemize the patients with necrotizing enterocolitis both were considered systemic manifestations and data of laboratory and instrumental study. Evaluation of immunological parameters were based on estimation of humoral and cellular components of specific immunity and body nonspecific defense factors: phagocytic activity of blood neitrophils, particularly phagocytic count, phagocytic index, end of phagocytosis, ability of interferon production by blood leucocytes in vitro; the study of ultrastructure of peripheral blood eosinophils and basophiles was performed on ultra thin microtomes after double contrasting with Tesla BS-500 electron microscope.  

The second stage of the disease was estimated in 12 cases, in other 39 newborns 3rd and 4th stages.

The performed study revealed following patterns: the results of immunological investigations correlated well with the severity of the disease and intensity of immune processes in newborns. Relatively subtle immune alterations were observed in the 2nd stage of the disease, according to the  intensity of immunodeficiency  and the severity of  disease, reliable decrease of T-cell parameters and gradual depression of humoral immune system were observed. The highest immunotrophity was described in severe cases of NEC – complicated by diffuse peritonitis.

The dynamics of ultrastructural alterations of eosinophils and basophiles before and after treatment include data about mobilization of organism sensibilisation and immediate type of immune reactions, which has obvious prognostic value.

The objective of the research was to study age-related changes in diameter of erythrocytes. Studies have been performed on three age groups: newborns (umbilical cord blood), adults (22-30 years old) and elderly (75-89 years old). Based on the results, we can assume that the diameter of erythrocytes is greater in newborns than in adults. This is caused by  low content of K+ ions, wich leads to hydration, increases the size of the erithrocyte and diameter becomes greater. In elderly people the diameter of erythrocytes remains almost unchanged.

Candidosis (candidiasis, moniliasis) is a fungus disease of the skin, mucous tunics and internal organs. Primary candidosis occursmore seldom. It is mainly appeared as a secondary process the background of the primary pathology.

Candidosis is one of the famous fungus infections in Georgia. Most of all we meet candidal, vulvovaginitis, stomatitis, skin form, balanopostitis, candidosis among the fingers, paronychia and onychia. The disease is appeared most in women (60%) than the men (40%). Mainly their age is from 20 till 40 years. Duration of diseases hesitates from 2 weeks till 1 year. Chronic forms are dominated. From attending pathologies, the digestive system, diabetes mellitus, cholecystitis, diseases transmitted by sexual way, leukemia, allergic status has detained the leading place. From pathogenic factors, the main importance has disorder sexual life, permanent and irrational antibiotic therapy, constant utilization of Corticosteroids and other.

Lipopolysacharide (LPS) toxicity in D-galactosamine-treated mice, leading to blocked gene transcription, results from tumor necrosis factor (TNF)-a-induced caspase-dependent liver injury. Drugs inhibiting caspase family can prevent or even overcome the ongoing hepatic injury induced by TNF-a during sepsis, ischemia-reperfusion, or severe hepatitis. The main aim of the present study was to examine potential anti apoptotic effect of immunomodulatory drug Plaferon LB in LPS/GalN induced liver injury model. Experiments consisted in developing a LPS-induced hepatitis in mice, analysis of serum ALT activity in serum and spectrophotometric study of the caspase-3 and caspase-8 activities in isolated hepatocytes. Our study revealed anti-apoptotic effects of PLB by decreasing the caspase-3 and caspase-8 activities in severe hepatitis compared to PLB untreated group, and constitutes a new promising therapeutic strategy for acute liver disease involving uncontrolled apoptosis.

THE INFLUENCE OF ATOPIC DERMATITIS ON QUALITY OF LIFE OF PATIENTS AND THEIR PARENTS

The goal of the research was to study psychoemotional disorders in children with atopic dermatitis and to determine the necessity of forming a correct strategy for overcoming parents’ crisis. There were examined52 children aged 4 -18 years with atopic dermatitis. They had been registered at the Georgian Research Institute of Dermatology and Venerology and they at least twice had recurrence of the disease. Psychoemotional characteristics and quality of life of these patients and their parents was being studied via special questionnaire. Two types of the questionnaire were used: for children diseased with AD – Children Dermatology Life Quality Index (CDLQI) and dermatologic index of quality of life also for parents - Atopic Derrmatitis Family Impact Scale (FDFIS). Both questionnaires contained 10-10 questions and questioning took 15-20 minutes.  The study showed that atopic dermatitis strongly impacts life quality not only of a diseased child, but its parents’ too, significantly changes their psychoemotional indices, physical and social functioning, form of parents-child contact. I the children, there were marked behavioral and disciplinary problems, communicative difficulties in contacts with other children and adults. In the parents, besides general emotional problems there were marked prominent social problems. The attitude of parents toward the disease, a correct strategy for crisis overcoming have a great importance for remission continuation and improvement of life quality of a diseased child.

PREVENTION OF REACTIONS AND COMPLICATIONS CAUSED BY COMPONENTAL HEMOTRANSFUSION IN RHESUS-POSITIVE RECIPIENTS

Transfusion of erythrocyte mass with the aim of correcting anemia is the major stage of treatment in patients at hematological clinic. Therefore the problem of the prevention and struggle with hemotransfusional reanimations and complications is considered as topical ones for hematology hospital.

During blood transfusion ABO system and system of Rhesus factor, including five antigens are taken into account.

Many clinical physicians consider that Rh+ recipients are safe to be transfused by Rh-erythrocytes. By present study we have tried to show that not only such transfusion tactics are dangerous but also some problems can arise at transfusion to Rh+ recipients even of Rh+ erythrocytes.

We would like to prove, that the definition of phenotype due to the system of Rhesus donor and recipient at transfusion of erythrocytes, is of great importance.

We consider that:

1.            Definition of Rhesus-phenotype should be obligatory and included in examination of patient at his/her reception in the hospital.

2.            To create computer base for Rhesus factor-phenotype of donors and recipients.

This information should be round the clock accessible to the medical personnel.

Phenotype due to the antigens of Rhesus factor system of blood of the donor and the recipient is considered as one more step on the way to the safe hemotransfusion.

peculiarity of Heart rhyme variability in the children diseased with bronchial asthma

The objective of the work represented the study of sympathetic and parasympathetic regulation on the bases of the estimation of Heart rate variability during the attack of bronchial asthma.

The research covered 54 children aged from 6 to 16 diseased with bronchial asthma. The diagnostics of disease were held on the bases of the results analysis of bronchial asthma global initiative criteria, in vivo allergic pre- tests and outer breathing functioning research.  Persistent course of bronchial asthma of average severity was ascertained. The control group was completed by 35 healthy children of the same age.

On the received results it was worked out, that vegetative dysfunction signs are characteristic for the children diseased with bronchial asthma, in the calm state with privileged vagotone regulation and during the domination of sympathetic part vegetative influence. At the same time high frequency of atopic dermatitis risk and hereditary bronchial asthma were ascertained in the vagotonic children diseased with bronchial asthma. In contrast to this group in vegetative regulation sympathicotonic children the specification of the intra-natal period and provocation of the disease by psycho genetic factors and   meteorolability were revealed.

THE REMOTE RESULTS OF ORGAN-SPECIFIC TREATMENT IN PROGRESSIVE HIGH MYOPIA

For about 30 years we  have been using melitesed  embryonic sclera as a transplant for surgical treatment of different diseases of the retina in our clinic. There was icrease of correct and uncorrected visual acuity after this operation. Perypapillary area of the retina was observed by digital photography of the eye fundus 1,3 and 7 years after scleroplastic operations. The appearance of additional vessels from visual nerve disk was registered 3 years after the operation.

THE FINDINGS OF TREATMENT OF AMBLYOPIA

Patients with amblyopia were treated by the method of noninvasive electrostimulation and contac lenses.

The findings show a high efficiency of this combination. There is no pain and no negative psychological effects for the patient. It is important to say that the result of the treatment depends on the type of amblyopia, initial visual acuity, compliance and the age when the treatment is started.
