THE PREDICTIVE VALUE OF THE CRP, FIBRINOGEN AND ESR IN ISCHEMIC HEART DISEASE

The aim of the study is to establish the role of inflammatory markers in development of acute coronary sindrome.

Materials and methods: 72 patients were investigated (age 65,7±12,1). They were divided into four groups: I group - patients with stable angina I-II functional class. II group - with stable angina III-IV functional class, III group – with unstable angina and the control group. The following analysis were studied: serum lipids- by BIOLABO tests (France), fibrinogen-by Rutberg gravimetric method, CRP- by ”Human” latex test, Erythrocyte sedimentation rate.

Results: The difference of C reactive protein, fibrinogen and ESR indexes between I, II, III and IV group was confident (p<0.01), VLDL, TG was significantly high in I, II and III groups in comparison with IV group, but there was not differences between I, II and III groups.

Conclusion: According our research dates there is evidence that  inflammatory proteins (CRP, Fb, ESR) are more informative markers  for atherosclerosis disease complications than lipid spectrum changes. In the same time the fibrinogen  level was more correlated with IHD complications than CRP and we suggest that this predictive marker is more valuable then others.

CHLAMYDIAL INFECTION, AS THE REASON OF UROGENITAL INFLAMMATORY DESEASES IN WOMEN

The goal of investigaition was the study of  urogenital deseases in women with Chlamydial infection in Gonorrhea department of Research Jnstitute of Dermatologe and Venereology last 5 year 1391 patients were investigated. In complex of studies  was included serological, bacterioscopic and cultural methods of investigation. 1122 (80,8%) patients were revealed. Chlamydial infection was diagnosed in 344 (28,15%) from them monoinfection in 210 (61,05%) case, mixed infection in 134 (38,95%) case. Clinical diagnosis was endocervicitis in 285 (82,8%). Endocervicitis and urethritis together in 21 (6,1%) and thistoutethritis in 9 (2,6%). Vulvovaginitis in 6 (1,7%), conjunctivitis in 4 (1,2%) patient. The 19 (5,4%) patients was diagnost lesion of upper urogenital tract: adnexitis – in 10 (2,9), salpingitis in 5 (1,5%) pielonephritis – in 4 (1,4%).

Data obtained by histological, electronmicroscopical  and morphometric investigation were shown that acute once exercise induce discirculatory disorders in intraalveolar capillary wall of lung, it’s important that lung structural discompletation depend on the alveolar macrophages activation and collagen synthesis acceleration.

As it is know, in the process of aging of the body cell membranes including one of erythrocytes suffer changes. Changes their phospholipid composition, conformation of membrane protein and this, results in changes of cell functions and violates homeostasis of the whole body. The aim of our research is to find out changes of physico-chemical parameters of erythrocytes in persons of senior age. Our studies yield that as a result of decline in membrane cholesterol concentration, as well as their weak hydration that promotes increase in cell volume, deformability of erythrocytes diminishes among elderly people. Little swelling of erythrocytes in elderly people must be caused by lack of cholesterol and other physico-chemical changes in membrane, structural-functional modifications of cation channel in membrane and accumulation of ions Na and K in the cells. The last condition facilitates inter-erythrocytes accumulation of Ca2+ and can be a cause of activation of Ca2+-dependant scramblasa, exposition of phospatidilcerin on the surface of erythrocyte membrane, following hemolisis and elimination of erythrocytes by macrophages. Increased activity of inter-erythrocyte anti-oxidant ferments, SOD, GR and G-6-PDG in a compensatory reaction of the body induced by increased generation of free-radical forms of oxygen and preventing  premature hemolisis of erythrocytes.

COMPLEX ASSESSMENT OF FACTORS INFLUENCING THE LIFE QUALITY OF PATIENTS WHO UNDERGO CATARACT SURGERY

Purpose: to assess factors influencing on quality of life and vision before, during and after cataract surgery.

Methods: Samples were chosen among cataract patients (n=257). Age 45-65. The questionnaires (culturally adopted “VF-14” and developed “patient satisfaction”) were filled in at the beginning, 1, 3 weeks, 6 and 12 months after cataract surgery. On same time complete ophthalmologic examination was performed. Patients with their first cataract surgery were chosen. In 55, extracapsular cataract extraction (ECCE) with implantation of posterior chamber intraocular lens (PC IOL) was done. In 72 was done phacoemulsification with implantation of hard PC IOL. In 100, phacoemulsification with implantation of foldable PC IOL was performed. 30 were in control group.

Results:  The main complain on comfort condition in hospital was long time waiting (n=63). In patients presurgical education (with video and computer programs) about cataract gave better results of satisfaction despite of complications during surgeries (n=27). Significant improvement in VF-14 score and patient satisfaction were noted 1 week after phacoemulsification (III group) and 6 weeks after ECCE.

Conclusions: Main factor in output of successful cataract surgery is meeting patient’s expectations and not achieving 20/20 vision.

ENDOSCOPIC CORRECTION OF NON-CANCEROUS GROWTH IN THE LARGE

770 colonoscopy cases in total were performed in the Clinic for Coloproctology of the National Medical Academy. Totally 137 cases of polyps different in size, quantity and localization were diagnosed in 95 persons (56 m. and 39 f.) of  28/79 years age group.

48 morphological verifications were performed in the cases of small polyps, and no dysplasia was found. 46 endoscopic polypectomies were done. Histological structure of 42 polyps were studied: dysplasia was diagnosed in 8 patients and the only case of malignization was discovered in medium size (0,6-2,0 cm) neoplasias. 31 polyps were excised endoscopically. There was performed only one resection of the sigmoid colon.

Endobioptic tissues from large size (more then 2 cm) neoplasias showed 8 cases of tubular-adenomatous forms (6 cases of dysplasia), 4 tubular-villous forms with coexisting dysplasia, 2 malignant polyps with highly differentiated adenocarcinomas.

There were performed 2 resections of the transabdominal colon. The other 12 patients underwent endoscopic polypectomies.

89 polypectomies resulted in three cases of primary, and one case of secondary bleeding. All complications were suppressed by endoscopic coagulation.

The remote results (1 to 3 years) showed a high efficacy of the endoscopic approach. There was observed one case of recurrence and one malignant transformation.

Endoscopic correction of benign colon polyps (despite their size and localization) is a radical measure in colorectal cancer prevention.

Micromorphological observations have shown that against the background of vasopressin injection the decrease of the functional activity of the hypothalamo-hypophyseal neurosecretory system takes please at the initial stage of development of a conditioned two-way avoidance reflex. It is possible to consider that it appears to be a compensatory-adaptive reaction of this system on the injection of  exogenous vasopressin.

STUDY OF NON-TRADITIONAL BRONCHODILATATOR FUROSEMIDE

EFFECTIVNESS IN PATIENTS WITH BRONCHIAL ASTHMA

To study the effectiveness of a non-traditional bronchodilatator furosemide in patients with bronchial asthma is very important along with the traditional bronchodilatators. The mechanism and efficacy of non-traditional bronchodilatators has been, however, less explored. The aim of our study is to determine the efficacy of furosemide utilization via inhalation in patients with moderate and severe forms of bronchial asthma. A controlled study was performed in patients (10/70 years old) suffering from bronchial asthma.

Utilization of furosemide via inhalation was rather effective than pre-inhalation using physiological solution. A controlled study was conducted at the Centre for Asthma, Allergy, and clinical Immunology with 20 hospitalized patients. The research was performed by simple method of randomization.

Patients’ condition was determined before and after the inhalations of furosemide and physiological solution.

After furosemide inhalation, sputum secretion and other symptoms were improved in 10 patients out of 13 - they felt better.

It became clear that in the case of placebo control, there were only 2 patients with improved condition out of 7.

According to our data, furosemide utilized through inhalation definitely revealed a positive treatment impact. Thus, the further investigation should be performed for its complete estimation.

The relationship between the duration of the intraocular pressure and different neuroendocrinal pathologies is established by clinical observations and experimental research. There is no similar opinion about the pathogenesis of glaucoma. One of them is the vascular theory.   Because, glaucoma represent the eye‘s vascular, ischemic disease, it is interesting to investigate renin-angiotensine system in patients with open angle glaucoma.

Our study was conducted in 71 patients (38 males and 33 females) with diagnosed open angle glaucoma and 71 persons too for control group. Their age ranged from 45 to 70 years old.

Their examination included ophthalmological and hormonal research. To evaluate the function of renin-angiotensin system, we measure the concentrations of  plasma renin, angiotensin I converting enzyme (SACE) and angiotensin II in the plasma of patients and control groups by radioimunassay method. The level of renin was increased and the concentrations of SACE and angiotensin II was decreasing. There was no difference between the gender and hormones levels. We have not seen any relationship to arterial pressure too.

These findings confirm the role of the renin-angiotensin system in IOP regulation.

THE EFFECTIVENESS OF BRONCHO-MUNAL-P IN CHILDREN SUFFERING FROM BRONCHIAL ASTHMA

The aim of our study was to find the way and methods for immunorehabilitation of children suffering from bronchial asthma. For immunorehabilitation we have used a medication produced in Slovenia - broncho-munal-P. The effectiveness of treatment was evaluated by studying the immune status before and after the treatment.

In the study we have included 23 children aged 3 to 15 years with diagnosed bronchial asthma. Prophylactic treatment was conducted in the phase of remission with usual dosage.

The results of immunological analysis revealed that the amount of T lymphocytes and CD8+ cells had raised in 20 cases, of the CD4+ cells - in 15 cases, B lymphocytes - in 16 cases after prophylactic treatment. 8 months later the immune status stabilized in all the cases.

We can claim that broncho-munal-P is an effective medicine for the prophylactic of bronchial asthma.

ATOPIC DERMATITIS IN CHILDREN

Atopic dermatitis is a chronic, recidivating, inflammatory disease which is characterized by intensive itch, papillary rash and manifested lichenification. It also includes other symptoms of atopy.

We have studied 60 patients with atopic dermatitis aged 0 to 14, who undertook 4 different types of treatment:      

1.   Eliminative diet.                                                                                                         

2.   Correction                                                                                                                           

3.   Skin treatment                                                                                                        

4.   Prevetional activities.

During the exudative form of atopic dermatitis corticosteroids were prescribed with great precaution. 38% of our investigated patients achieved complete remission, in 42% there was noticed some improvement of skin condition and only in 20% of diseased children rhinitis and asthma developed.

THE PECULIARITIES OF THE LUNG RESPIRATORY PART AND CARDIOMYOCYTES AFTER THE CHRONIC PHYSICAL TRAINING IN EXPERIMENT

In the purpose of determination of the  functional morphology of the lung respiratory part and cardiomyocytes  after  the chronic  physical exercises experimental studies on 80 albino rats were carried out. Experience take place in rotated tredbane for 2 hours twice in the week during 2 months. Frequency of rotation was 19 rot/min.

The results of our investigation gave us an opportunity to conclude that after the chronic physical training decompensate respiratory acidosis and deep hypoxia develop, destruction in the lung aerohematic barrier and cardiomyocytes is observed. That means that the trophic of the myocardium is also disturbed. All these events are expressed in establishing of the «cardiopulmonary» reaction. Thus the physical training is not adequate for an organism.

The community of the pelvic cavity venous plexuses actually forms a voluminous enough venous basin – “venous depot of the pelvis”.  Experimentally we achieved a rapid transference of the blood from this basin into the general circulation of the organism.  At the same time there were determined the speed of blood transfer and efficacy degree of such intervention which expressed itself in an increase of blood pressure in the vessels of the great circle of blood circulation.

The aim of our study was to investigate the peculiarities of lipid metabolism and insulin resistance in patients with metabolic syndrome (MS) and impaired carbohydrate metabolism of various degrees. We selected 128 overweight patients from Imereti Region. MS in the patients was determined by WHO classification, insulin resistance was evaluated by HOMA. Total cholesterol, triglycerides, HDL, LDL, VLDL, and atherogenic indices were measured. The number of MS features did not depend on the state of carbohydrate metabolism. b-cell function and insulin sensitivity decreased with the increase of the impairment of carbohydrate metabolism; respectively, insulin resistance was increased. Lipid metabolism in patients with normal glucose tolerance is significantly less impaired than in patients with impaired glucose tolerance and DM2. There were not observed any significant differences in the parameters of lipid metabolism of the last two groups.

INTRASTROMAL RINGS IN KERATOCONUS TREATMENT

Purpose: to evaluate effectiveness of the use of Intrastromal Corneal Rings for the stabilization of keratoconic process and vision correction in mild and moderate keratoconus

Methods: the 54 cases of Keratoconus   and 2 cases of  myopic astigmatism. 44 patients were studied.  The patients undergone full ophthalmologic examination including corneal topography on Orbscan II as a modern, most informative method for diagnosis/treatment. Corneal topography maps were used as guidelines in the operation procedure for ICR localization. 1 or 2 (when needed) PMMA ICR were implanted in 5 mm zone, in 2/3 of corneal thickness, taking into consideration the localization of Keratoconus head and the linear board deformity spread on the posterior surface fixed by Orbscan II. The follow-up  period  overed 22  months.

Results: Mean uncorrected visual acuity ( UCVA ) was improved from 0.06 to 0.6. Mean Best corrected visual acuity ( BCVA) was improved from 0.4 to 0.7. The findings showed reliable positive changes of visual improvement ( p<0.001) Keratometric data for both anterior and posterior surfaces were diminished minimum by 3.0 Diopters, showing also statistically reliable changes, as well as irregularity ( by Orbscan II ) for 3 mm and 5 mm zones. The data mentioned above were absolutely stable and were not changed during the follow-up period.

Conclusion: ICR implantation seems very effective for stabilization of cornea condition as well as the correction of visual acuity in Keratoconic patients. The procedure is safe and simple to conduct and seems to be a good enough alternative to Keratoplastics.

The aim of our study was the investigation of the peculiarities of LPL activity and lipid metabolism during IHD in different age and sex groups. 52 patients with IHD (30 females and 22 males) have been investigated. Total cholesterol, triglycerides, HDL, LDL, VLDL and atherogenic indice (AI) were measured. Age significantly inversely correlated with LPL activity (r=-0.5306, p < 0.001). A significant correlation was observed between age and HDL (r=-0.2928, p = 0.035), LDL (r = 0.3690, p = 0.007), AI (r = 0.2954, p = 0.033). There was no significant difference in LPL activities between sex groups. Significant differences were observed in serum LDL (p=0.042), VLDL (p=0.015) and TG (p=0.017) levels.

ACTIVITY CRITERION OF HEMORRHAGIC VASCULITIS IN CHILDHOOD

Vasculitis is a descriptive term associated with a heterogeneous group of diseases that results in inflammation of blood vessels. The pattern of vessel involvement is highly variable, leading to innumerable clinical presentations. The most common vasculitis of childhood is Hemorrhagic Vasculitis (HV). The spectrum of the expression of Henoch-Schonlein purpura may vary from minimal petechial rash to severe gastrointestinal, renal, neurological, pulmonary and joint diseases.

The aim of the study  was to assess clinical manifestations of  HV using  Vasculitis Activity Scores, thus assessing process activity level and organ distribution of the disease. 35 Patients (24 – initial presentation, 11 – relapse) with the established diagnosis of HSP during last 5 years were observed at the Children’s Central Hospital. 7 patients experienced minimal activity (2-4 scores), 15 – moderate (5-7 scores), 13- with high activity process.

Thus, the  modern approach to vasculitis management gives the opportunity not only to  assess process activity but also in choose an appropriate treatment modality by the principle– the aggressiveness of a disease should match the aggressiveness of its treatment.

REACTION OF HYPHOTALAMIC BIOLOGICAL  AMINES AND SOME HORMONES ON VIBRATION  PATHOLOGY

The experiment was held on 20 grown-up male rats which experienced 1 hour vibroseance every day during 2 months (Parameters: frequency-50 hz, amplitude- 3mm). Modulation was fulfilled in the kind of common vibration.

 The rats were researched after 2 months after decapitation in conditions of general narcosis. By means of high-performance liquid chromatography method the quantity of biological amines (norepinephrine, dopamine and serotonine) in hypothalamus was defined, and the concentration of sex (testosterone and estradiol ) and thyroid hormones (T3 , T4) in the blood serum was also defined  by immunoferment method. As a result of the research it was established that: the quantity of estradiol was a little bit more than the upper limit of the norm (N 15.0-60.0 ng/ml ) and was equal to 63,6ng/ml, and the rate of testosterone fluctuated within the norm 8,11-13,8ng/ml    (N 8,0-22,0 ng/ ml).  The concentration of  thyroid hormones (T3   -  3,2-4,5pg/ml,    T4 – 1,0-1,7 ng/dl ) did not differ from the same indices of intact animals (N T3 1,37-7,1 pg/ml, T4   0,3-3,6 ng/dl ).

In   rats the expressed alterations of the concentration of norepinephrine, serotonin and dofamine was ascertained after the vibropathology had been formed. The rate of norepinephrine was increased by 62% (N »1024 ng/gr of tissue, the time of pathology 1448±214 ng/gr of tissue ). By 60% increased the rate of serotonin too  (N» 804 ng/gr of tissue, at the time of vibration pathology 1186±73 ng/gr ). The most perceptible alteration was ascertained during the definition of dofamine and the significant tendency of its decrease by 83% was distinguished (Norm» 577 ng/gr of tissue), and after 2 months of vibroseance its concentration fell down to 82±18 ng/gr of tissue.

CAUSES HINDERING BINOCULAR VISION RECONSTRUCTION DURING CONCOMITANT INWARD STRABISMUS

We analyzed the results of complex treatment of 136 patients aged 3 to 6 who are affected by non-accommodative alternated inward strabismus. From the mentioned contingent, there were separated 5 groups of risk-factors hindering the development of binocular vision. The control group without risk factors was taken separately.

The analysis of treatment results showed that binocular vision reconstruction is hindered mostly due to the strong deep amblyopia that is present before the  treatment beginning, and the vertical component which remains after the correction of deviation. Not strongly, but confluent work of the eyes is still hindered by inward strabismus developed from the moment of birth or before the age of 3 months, also by a deviation higher than +450 and its late treatment.

We have investigated histological, histochemical and ultrastructural peculiarities of the kidneys tissue of adult and newborn donors with a 60-minute ishemia. The results show deep irreversible structural and functional changes that enables us to determine that such kidneys are not suitable for transplantation.

DIFFERENTIAL TACTICS FOR PRESCRIPTION OF METABOLIC MEDICATIONS DURING IMMUNE STATUS CORRECTION IN INFANTS WITH LIGHTS FORM OF LOCAL INFECTIOUS INFLAMMATORY DISEASES (LIID)

Depending on the course of local inflammatory processes infants ware divided into 3 groups: I group – normal infants on natural feeding. There were 27 of them. II group – 20 diseased infants. III group – 26 patients born at the end of pregnancy with nephropathy of 1-2 degrees. The control group consisted of 20 healthy infants.

Along with antibiotics, the preparations improving metabolic processes were prescribed to the patients of all the groups for 70 days. For this purpose there was prescribed vitamin mixture of a physiologic doze (À, Å, Ñ, Â1, Â2, Â6, ÐÐ). Patients of the 2 group were additionally injected with immunostimulator quartsetin (15-20mg/kg per day). After treatment the immunity indices (IgE, Sik, Nst-test) didn’t normalize only in the III group of patients. Therefore, sick infants of this group refer to the group of  high risk.

IMPACT OF CORTICOTROPH HORMONE OF ADENOHYPOPHISIS ON ADRENAL GLAND DURING VIBRATION PATHOLOGY IN THE EXPERIMENT

The target of our research is a condition of hormone system of the adrenal gland and reaction of the auricular natriurezal polypeptide on vibration pathology.

The research was held on 20 grown-up white male rats. The model of vibration pathology was used. The vibroséance lasted for 1 hour every day during period of 2 months. The animals were killed by decapitation on the 20th, 40th and 60th days. In the blood plasma the concentration of Adenocorticotroph hormone (ACTH), cortisol and auricular natriurezal polypeptide (ANF) was being defined. All the initial values were considered to be 100%. On the 20th day after the trail being started the increase of ACTH concentration by 160% took place), on the 40th day decreased to 137%, and on the 60th day decreased to 92%. The concentration of cortisol on the 20th day was 205%, on the 40th day – 160% and on the 60th day decreased to 132%, and maintained on this level. The concentration of auricular natriurezal polypeptide (ANF): on the 20th day increased to 156%, on the 40th day it was 128%, and on the 60th day decreased to 89%. From the received data it follows that the vibration impact in rats causes alterations, which are revealed in quantitative alterations of adenocorticotroph hormone (ACTH), cortisol and auricular natriurezal polypeptide (ANF). The regulation of corticotroph function of adenohypophisis is fulfilled by negative back connection between the adrenal gland and hypophysis. This mechanism is not totally violated, as the increase of AKTH concentration causes reciprocal activation of cortisol and accordingly it  resultse in decrease of AKTH concentration with decrease of synthesis of cortikotrophin-releasing-factor in the hypothalamus with negative back mechanism. The neurosecretory cells producing ANF are in the central nervous system, hypothalamus, hypophisis, and adrenal gland. From these structures the ANF is overthrown to blood under the impact of nococeptor impulsation. The alterations of ANF concentration depend on the state of an organism and its needs.

ASSESSMENT OF CONDITION OF CEREBRAL AND OCULAR VESSELS BY MODERN METHODS OF RESEARCH (PRELIMINARY DATA)

Cerebrovascular abnormalities often cause significant vascular changes in orbit. The purpose of our study was CT evaluation of a. ophthalmica in case of various cerebrovascular abnormalities.

The role of imaging technologies in exploring the anatomical composition of the eye vascular system is huge. We have used CT and CT angiography, because it makes possible to visualize structural changes both of cerebral and ocular systems.

According to the results of our research we can suppose that constriction of cerebral arteries can lead to the artificial increase of blood delivery to the a. ophthalmica. Our study had been designed as much as possible to ensure a fair and objective evaluation of this new method.

CLINICO-IMMUNOLOGICAL EVIDENCE OF LOCAL INFECTIOUS IMFLAMMATORY DISEASES IN INFANTS AGAINST THE BACKGROUND OF PERINATAL LESION RESIDUAL OF THE CENTRAL NERVOUS SYSTEM

Out of 487 infants under observation who had LIID were selected 15 ones born to mothers with pathological course of pregnancies and deliveries, and with the premorbid background of perinatal lesion (PL) of CNS - they were included in the I (basic) group. The control (II) group consisted of 18 patients born to mothers who had had only pathological course of pregnancy. In the control group, there were 20 healthy infants.

The assessment of cell-bound immunity in all the observed groups was conducted via determination of the absolute quantity of lymphocytes and phenotype (ÑÄ3+, ÑÄ4+, ÑÄ8+, ÑÄ14+, ÑÄ16+ cells) in 1ml of blood, and also was studied the reaction of blasttransformation  of leukocytes.

The results of the research showed that the parameters of cell-bound immunity undergo definite reliable changes as compared with the healthy infants and patients from the II group. Apparently, these negative shifts are preconditioned by the presence of an additional premorbid background – residual of PL CNS.

On the basis of the carried out researches and given literature, the renal dynamic scintigraphy at checkup of patients with chronic renal insufficiency is almost the unique objective method of analysis  which is not connected to the risk, irrespective of a severity of condition. The specified research gives the information on functional reserve potential of each kidney, that is extremely important for realization of a rational treatment for the given disease.

Accordingly, the renal dynamic scintigraphy with I131 hippuran provides information on the functional condition of a kidney, quantity and distribution of a functioning parenchyma in kidneys, secretory-descending function of kidneys that facilitates to diagnosing, finding out the reason of disease and a choice of a method of treatment.

At the end of pathologic process the skin of affected areas becomes sclerotic. Very often pathohistologic processes involve entire connective tissue. At the same time hair are located in form of brushes. Surgical and conservative treatments are in use.
