THE CLINICAL EFFICACY OF OLYCARD 60 RETARD AT STABLE ANGINA PECTORIS

The study aimed to assess the antianginal efficacy of the medicine Olycard (Isosorbid-5-mononitrat), produced by pharmaceutical firm Solvay Pharma, among the patients with stable angina. The study involved 50 patients of various ages with stable angina pectoris (II, III and IV func. class) who had been diagnosed with stable angina about 3 month before the study and whose rate of anginal attacks was 5 or more per week. The patients were treated with Olycard 60 Retard for 2 month period.

The study demonstrated significant positive results of Olycard 60 Retard in the patients with stable angina. It reduced: the rate of anginal attacks (from about 10,4 to 2,3 weekly), the quantity of Nitroglycerine tablets used weekly (from about 12,5 to 2,2), the functional class of 50% of the patients. 92% of the patients announced the improvement of their life quality.

The significant positive results of our study can be explained by a number of characteristics of Olycard 60 Retard: high bioaccessibility (100%), unique pharmacological form, good endurance, low risk of development of tolerance to the medication, rebound-syndrome, only one active substance in serum, high efficiency, convenience for using.

Nowaday,s in medicine a great attention is payed to the use of biogenic stimulators. One of such materials is the amnion (AM).We have fixed on one of the most economic ways for  preservation of a biological tissue - preservation in natural bee honey. We have studied EPR signals of an intact AM and of the AM preserved in various environments. From the received data it is possible to conclude that AM keeps it’s viability during the conservation in 100 % bee honey.

MORPHO-FUNCTIONAL CHANGES OF  PLACENTA  DURING CYTOMEGALOVIRUS INFECTION

Viral infections still remain an urgent problem of contemporary medicine. Cytomegalovirus [CMV] is one of the most common viruses with the ability for fast growth and  propagation, and the tendency for a prolonged persistence in the human of organism.

Maternal and fetal components of 40 placentas obtained from full-term pregnancies immediately after vaginal delivery or cesarean section have been examined microscopically.

Age of parturients fluctuated from 17 to 38 years, 29,3±1,4 on average; the number of gestations 1-5, the number of deliveries 1-3.

In all the samples of human placentas affected by cytomegalovirus infection obtained from full-term pregnancies, the picture of focal productive inflammation is evident in both maternal and fetal components of the placenta, being expressed in syncytial trophoblasts, amnyocytes, and vascular endothelium. The process is  characterized by the presence of  huge  mononuclear cells in the basal  membrane; hyperchromatic nucleus surrounded with  a light circle, the so- called  ‘’owl’s  eye’’  and basophilic inclusions.

ESTIMATION OF  THE IMMUNE STATUS OF CHILDREN WITH MINIMAL CHANGE NEPHROTIC SYNDROME (MCNS)

The presence of a serous immune component in the pathogenesis of MCNS points to the need for estimation of the organism immunocompatibility under complex treatment in children. The studies have shown a reliable increase in the concentrations of IgE in blood serum, the intensity of which was connected with a hormonal dependency of the disease. It also revealed imbalance of the other immunological factors: the activation of immunoglobulin synthesis and increase of the percent of B-lymphocytes under the reliable suppression of phagocytosis and interferon system, as well as of the amount of T-lymphocytes and their subpopulations. The questions about the need for the use of immunomodulating preparations in complex treatment of children with MCNS for the increase of treatment quality are being discussed.

MORPHOLOGY OF HAIR FOLLICLES DURING VARIOUS FORMS OF ALOPECIA AREATA

The objective of the research is to reveal and compare to each other the forms (localized, extensive and familial) of  alopecia areata. The research has been conducted with 12 patients diseased by alopecia areata (4 in each group) and with 3 individuals of the control group. In the skin of 3 patients of the 4 having an extensive form, mesenchymal-inflammatory syndrome was noted – less in comparison with the acute stage, but more expressed as compared to the other forms. Via transmissive electron microscopy, in the remaining hair follicles of these 3 patients, besides the necrobiosis and apoptosis of epitheliocytes, attention is attracted by the emergence of the cells with so called “dark” cytoplasm. In the nuclei of such cells, there takes place chromatin decondensation and transformation of the ring-shaped nucleoli into the nucleoli of nucleolonemic texture characteristic to the standard. We considered worthy of attention only changes developed close to the skin surface, in the epithelial cells of the outer socket of a telogenic hair root. During the study of Ag-NOR proteins via staining method, in the nucleoli of the epitheliocytes in this area, numerous Ag positive zones located closely to each other form a chainlike structure corresponding to the nucleolonemic texture. Such changes develop also in the endotheliocytes of the hemocapillaries of hair papillae. In the patients with localized and familial forms, structural alterations occupy mid-place between the extensive form and control.

COMPARATIVE  EFFECT OF ETIOTROPIC AND IMMUNOMODULATING THERAPY OF UROGENITAL CHLAMIDIOSIS

There was offered a scheme for the treatment of urogenital chlamidiosis based on etiotropic therapy (Sumamed and Tarivid) with the use of the enzyme (Wobenzym) and immunomodulating preparation (Plaferon-LB and Phenowine). The results of the treatment were valued for etiological recovery (i.e. bacteria carrier-free) and reduction of the duration of inflammatory symptoms in patients. The efficiency of the treatment in this group formed 93,2%, when under traditional therapy with only antibiotics the results did not exceed 80%. The efficacy of the offered scheme was confirmed by the pregnancies which occurred in 22,7% of cases after complex treatment against 10,0% 0 in the group of traditional therapy.

A stable increase in intraocular pressure was achieved by the injection of chemotrypsin into the posterior chamber of the eye in rabbits. The high initial level of elastic curves and of their break points, the increase in the coefficient of outflow easiness of intraocular liquid were revealed. There was shown participation of two factors in the mechanisms of chemotrypsin glaucoma origin is  the factor of difficulty of intraocular liquid outflow and that of the disturbance of the local regulation of intraocular pressure.

PARTICULAR FEATURES OF THE EXPRESSION OF IMMUNOHISTOCHEMICAL MARKERS IN BREAST CARCINOID

Study is based on the investigation of the materials 7 cases of breast cancer obtained, after surgical extirpation made in the department of mammology of A.Gvamichava National Cancer Center. The study was carriedout by the of the use histological and immunohistochemical methods.

For immunohistochemical investigation we used mono- and polyclonal antigens: chromogranin, estrogen and progesterone receptors, oncoproteins HER-2/neu and  proliferation marker Ki-67.

Chromogranin expression was found only in the carcinoid, not in cancer cases. Therefore, this marker should be used for differentiation of carcinoid from other tumors.

In breast carcinoid the expression of the estrogen and progesterone receptors is high, but expression of the proliferation marker Ki-67 (mitotic activity of tumor cells) is low and oncoproteines HER-2/neu are not revealed.

We suggest that our findings are interesting not only as breast cancer diagnostic means, but also for the formation of the treatment concept.

LAPAROSCOPIC REPAIR OF VENTRAL AND INCISIONAL HERNIAS

We retrospectively reviewed the first nine laparoscopic ventral and incisionsl hernia repairs performed at Z. Tskhakia Tbilisi Municipal Thoraco-abdominal Clinic from 2001 to 2005.

Laparoscopic repairs were performed using Biomesh made by W.L. Core and Associates (GORE-TEX Soft  Tissue Patch, GORE-TEX  DUALMESH Biomaterial and GORE-TEX DUALMESH PLUS Biomaterial with CORDUROY Surface), with a minimum of 3 cm overlap circumferentially with normal fascia and secured with spiraltacks at every 1-1,5 cm and transparietal sutures.

All adhesions on the parietal debect are released, using scissors, and whenever possible we attemped to reduce the hernia sac.

In tree cases were performed laparoscopic simultaneous operations: laparoscopic cholecistectomy.

The mean follow-up was 26 months with a range of 2 to 44 monts. There was no recurrence in the group. There were four seroma cases in our group, wich disappeared without treatment. By this date, no chrinic infections, bowel obstructions, or enterocutaneous fistula have been reported. 

In conclusion, our results are encouraging, and suggest that  laparoscopic ventral and incisional hernia repair is a safe, feasible and effective alternative to the open techniques.

DETECTION AND CORRELATION OF  HPV-INFECTION IN PATIENTS WITH CIN AND AGUS

Objective: The aim of our study was to detect and correlate HPV-infection in patients with  cervical intraepithelial neoplasia (CIN) and atypical glandular cells of undetermined significance (AGUS).

Design: prospective study.

Setting: Zhordania Institute of Human Reproduction, Tbilisi, Georgia.

Material and subjects: We studed cytological materials of cervix utery of 334 patients, chose and classified 92 patients with condylomatosis. These patients were selected for three groups: I group– Cervical Intraepithelial Neoplasia (CIN) – n= 53. II group – Atypical glandular cells of undetermined significance (AGUS) – n=23. III group -CIN + AGUS –n= 16.

Methods: Pap-smear test, colposcopy, morphometric research, statistical research.

Results:  HPV was detected in 84,9% of the first group (CIN)-p<0,05, in 4,3%- of the second group (AGUS) and in 18,75% -of the third group (CIN+AGUS).

POSTPRANDIAL PLATELETS FUNCTIONAL MORPHOLOGY IN CHD PATIENTS

Aim of our study was to determine the morphological changes of the blood platelets in atherosclerotic women, and to reveal the relation between lipids and fibrinogen.

Results: in the control group, after nutritive load, there  was found solitary big platelets with pseudopodia; part of them was degranulated, without a notable increase in aggregation. LDL and fibrinogen rates were significantly increased, and HDL cholesterol level was decreased. In this group big platelets were degranulated and some of them had pseudopodia and aggregations. Only active platelets have IIb/IIIa.

Based on our investigation we can conclude that nutritive load in atherosclerotic women increase risk of thrombogenesis and raise a question about diet.

We aimed study the influence of Plaferon-LB and lazer therapy on the decrease of recurrent LOR-organ diseases.

15 children with diagnosed chronic tonsillitis, in the age from 6 to 10 years were included in the study.

In all cases the status of local immunity was determined (IgA and G in saliva), together with microbiological status of pharynx secretion.

The results revealed that the treatment by Plaferon-LB and lazer therapy, improves local immunity and microbiological status of children suffering from chronic tonsillitis.

METABOLISM OF MICROELEMENTS - ZINC AND COPPER - IN CHILDREN WITH ACUTE OBSTRUCTIVE BRONCHITIS AND INTERMITTENT BRONCHIAL ASTHMA

On the basis of our study data we infer that during the acute phase of obstructive bronchitis and intermittent bronchial asthma, the metabolism of microelements – zinc and copper is disturbed.

In the serum the level of copper was elevated sharply in both types of the disease but especially in obstructive bronchitis. As regards the level of zinc, it was only modertly increased in patient with bronchitis and remained within normal range in those with intermittent bronchial asthma.

On the contrary, in the lymphocytes the amount of these microelements display tendency for diminuation and correlation between the level of zinc and copper also changed particularly, negative correlation between these two microelements, which is normal phenomenon in healthy subjects, was not observed in patients with intermittent bronchial asthma.

Thus, it may be supposed that these changes in metabolism of zinc and copper in children with obstructive bronchitis and intermittent bronchial asthma in the acute condition is due to compensatory mechanism, but some deficit of zinc should be concidered too.

VENTRAL AND INCISIONAL HERNIA REPAIRS USING PROSTHETIC MATERIAL

Authors retrospectively reviewed the first ninety four cases of ventral and incisional hernia repairs using prosthetic materials. 85 cases were performed by open surgry and 9 by laparoscopic route. In 80 cases open mesh repairs were performed using prolen and polypropilen prosthesis (Autosuture, Ethicon, Ëèíòåêñ) and in 5 cases expanded polytetrafluoroethilene (eRTFE).The choice of prosthetic material was based upon availability and sugeon’s preferens. Laparoscopic repairs were performed using Gore-tex Soft Tissue patch, Gore tex DUALMESH Biomaterial and Gore Tex Dualmesh Plus biomaterial with CORDUROY Surface. Peculiar methods of surgery included classical methods and also several modifications proposed by authors for different cases. Criterias of the choise method were size and localisation of hernia defect, age of patient, will of patient and surgeon’s experience.The hernias size ranged from 1,5x1,5 cm to 22x35sm. Median follow-up was 22 mounths for open mesh repair and 12 mounths for laparoscopic repair patients. The rate of reccurence was 2,3% for open mesh repair and 0% for laparoscopic repair. The mean hospital stay in laparosocpic series was 1.1 day and 5,4 day for open mesh repaire.

As a conclusoin authors state that the use of prosthetic materials in  tension free ventral and incisionsl hernia repairs has siginifucntly diminished reccurence rate and period of social rehabilitation.

We have studied EPR signals of intact sclera preserved in various enviroments: in natural honey and in MEM enviroment.

The aim of our study was to reveal the most frequent complaints, of the adolescent, the difference in complaints between males and females, between pupils of payed and charge free schools, by making special questionnaire which included the problems of health, different addiction (smoke, alcohol, drugs), lone  psychological paculiarities, behaviour pathologies, sexual development e.c.

400 adolescents in the age from 15 to 17 years got acquainted with the questionnaire. The testing was anonymous.

The results revealed that the changes of economical status of the population reflected on adolescents by worsening health condition, sexual and psychological development. To solve this very important problem we regard necessary to restore consultative rooms, non-ductiles regular prophylactic missions, organize preventive and rehabilitation services.

Nitric Oxide is known as one of the most powerful vasodilatators for many vascular regions.

Investigation of neurogenic vasodilatation of lingual artery has shown that nitric oxide is not responsible for this vasodilatation and Calcitonin Gene Related Peptide (CGRP) released in response to stimulation of non-adrenergic and non-cholinergic nerves, caused relaxation of lingual artery and that in its turn is mediated by CGRP 1-receptors.

In vitro investigation of endothelium-denuded and intact segments of rabbit lingual artery by means of mechanotronic device has confirm strongly pronounced role of calcitonin gene related peptide in vasodilatation of mentioned artery and possibility of derange this debugged mechanism by means of hydroxyl radicals. As a generator for hydroxyl radicals we used the Fenton’s reaction.

INSOMNIA

Lots of scientists infer that 28-45% of people have insomnia. Our objective is to learn the biochemical aspects of insomnia and its links to other pathologies and to find the way of its prevention. We’ve used Hamilton’s questionnaire and revealed that out of 50 persons involved in the experiment 40 (80%) have disturbed sleep, 18(45%) have difficulties with sleep, 7(17%) wake early, 15(30%) have severe depression. Serotonin low concentration in the blood plasma had 15(30%), average – 10(20%), and high – 25(50%).

Thus, depression is the main factor for most people having insomnia. But general sleep reduction, loss of interest and capacity for work, psychological alarm are also significant characteristics of insomnia. Persons who sleep well have serotonin low concentration in blood plasma. Thus, during insomnia we need to find out its causes, for its treatment is not only symptomatic, but it’s also essential to know the etiology. The prevention means include not only medical  biochemical investigation, but psychorehabilitanion methods as well.

comparative estimation of the effectiveness of the invasive methods of the diagnostics of helicobacteriosis

Comparative estimation of efficiency of invasive methods has been performed in the diagnostics of helicobacteriosis. 147 biopsy material taken from different parts of the patients stomach and duodenum with gastroduodenal pathology associated with Helicobacter pylori have been investigated by bacteriological cultural, biochemical uridrosis, morphological histological and cytological methods. Cultural and cytologic methods showed maximal sensitivity and specificity, but the latter is more accessible and economic and shorten the time of diagnostic result, at least from 3 days to a few hours.

Accounting constant presence of H.pylori in antral division of stomach, it is enough to investigate one biopsy material, that reduce traumogenocity of invasive diagnostics of helicobacteriosis.

LIPID SPECTRUM AND INFLAMATORY MARKERS IN PATIENTS WITH CORONARY HEART DISEASE AND HELICOBACTER PYLORI INFECTION

The aim of this work is to study  the relation between atherosclerosis risk factores (lipid exchange, fibrinogen) and HP in  patients with CHD.

Materials and Methods: We selected 36 patients with CHD (proven by means of myocardial infarction documented in hospital and by ECG and veloergometere tests): 22 patients with CHD and Hp infection compared them with 14 patients without Hp infection from the control group (CG). Blood samples were colected for  each person after 12 hour fasting.Non-hemolised serum underwent the following tests: Total Cholesterol (TC), Tryglicerides (TG), High Density Lipoproteins Cholesterol (HDL-C). Fibrinogen in plasma was performed by Rutbergh  method. CRP by immunferment analysis. All patents underwent upper GI endoscopies (“Olympus”). Hp status was revealed by urease test of gastric antral and corpus mucosa specimens. Statistical Analysis -Continuous variables are presented as mean (M) ± standard deviation (SD). Student’s T test for data was used to compare values within subject groups; Statistical significance was defined as P<0.05. Correlation (r) were tested by Pearson  correlation.

Presented data shows the significant links between the structural and functional changes in the lipid range, indicators of acute phase proteins in case of atherosclerosis and with the presence of HP infection. Therefore, we think, that HP infection is a predisposing factor for atherogenesis, and in case of atherosclerosis - an aggressive factor, that may induce destabilization and exacerbation of atherosclerotic plaque.

Thus, HP patients with hyperlipidemia make up high risk group for prevention of atherosclerosis.

CHD patients with HP infection and increased indicators of acute phase proteins are the population under the high risk of possible exacerbation of atherosclerosis.
